眼眶粘膜相关性淋巴组织淋巴瘤组织学与影像学相关性分析

摘　　要：目的:分析眼眶粘膜相关性淋巴组织淋巴瘤(mucosa—associated lymphoid tissue lymphoma,MALTL)的临床病理组织学与影像学表现的特征,为该疾病的术前诊断、治疗提供帮助。方法:回顾性分析20例25眼眼眶MALTL的一般临床资料、影像学特点及病理特征。结果:发生于眼眶18例21眼,结膜2例4眼。术前进行CT、MRI和B超检查显示MALTL有特征性的改变。术后病理和免疫组化检查显示本肿瘤均为B细胞起源,21眼单型免疫球蛋白检测阳性,其中15眼κ阳性,6眼λ阳性;15眼CD45RA阳性,18眼CD45RO弱阳性,23眼CD20阳性,24眼CD3阴性。结论:眼眶MALTL与其他部位MALTL有相似的病理形态特征,可通过影像学检查得到早期诊断。Purpose: To analyze the relationship of the pathological and clinical features of patients with ocular adnexal mucosa-associated lymphoid tissue (MALT) type lymphoma, and to provide a suggestion of preoperative diagnosis and treatment of MALT lymphoma. Methods:To analyze the clinical, imaging and histopathologic features of 20 cases (25 eyes) of MALT lymphoma retrospectively in the department of ophthalmology. Results: Among the 20 cases (25 eyes), 18 cases (21 eyes)were located in the orbit, and 2 cases (4 eyes) were at conjunctiva. MALT lymphoma had typical characters in CT,MRI and B scan imaging, Histopathological examination showed that all of the 20 cases (25 eyes) were B cell lymphoma. Monoclonal immunophenotype of 21 eyes were positive, 15 eyes K positive,6 eyes λ positive. 15 eyes CD45RA positive, 18 CD45RO low positive, 23 eyes CD20 positive and 24 eyes CD3 negative. Conclusion: Patients with primary ocular adnexal MALT lymphoma had a similar pathological morphology with other locus. Through imaging we could get a preoperative diagnosis.
