摘 要:目的 :观察全反式视黄酸点眼对兔角膜新生血管的影响。方法 :32只日本大白兔随机分成A、B、C、D 4组 ,每组 8只兔 (8只眼 ) ,制成角膜碱烧伤模型。治疗组 (A、B、C组 )伤后即分别滴用 15 ,30及 6 0mg·L-1的全反式视黄酸滴眼 ,对照组 (D组 )滴赋形剂 ,裂隙灯观察角膜新生血管的生长情况并计算伤后第 6 ,9,12 ,15 ,18,2 1,2 5 ,2 8d的角膜新生血管面积。连续用药 2 8d后处死兔并取下角膜做组织病理学检查。结果 :伤后B组和C组角膜新生血管开始出现的时间明显较A组和D组延长 ,且角膜新生血管生长面积均明显较A组和D组减少 ,组织病理学检查显示CNV面积与角膜后膜和炎性细胞数均有明显相关性。结论 :局部应用全反式视黄酸对由碱烧伤引起的兔角膜新生血管的生长有明显的抑制作用。Objective: To investigate whether the all-trans retinoic acid (ATRA) topically applied can inhibit corneal neovascularization(CNV) in rabbits induced by alkali burn. Methods: Following the central cornea of right eyes were burned by 1 mol·L -1 NaOH for 30s,thirty-two Japanese white rabbits were randomly assigned to 3 study groups and a control group(each consisting of eight rabbits). The ATRA drops with different concentrations were applied topically to rabbit corneas three times daily for four weeks on each group: 15 mg·L -1 in group A,30 mg·L -1 in group B and 60 mg·L -1 in group C,and carrier solution was used in group D(control). Rabbits were observed under the slit-lamp biomicroscopy and the area of CNV was calculated on day 6,9,12,15,18,21,25 and 28. Then the rabbits were killed on the 28th day(d),and the corneas were taken for histopathological analysis. Spearman’s analysis was used to analyze the correlation between the number of inflammatory cells and the area of CNV. Results: The occurance of CNV in group A,B,C and D was (3.8±0.7)d,(5.4±0.5)d,(5.8±0.7)d and (3.5±0.8)d,respectively. The differences between both group C and D as well as group B and D were statiscally significant ( P< 0.05). Whereas the difference between group B and C as well as group A and D were not significant( P> 0.05).At set time points,the areas of CNV in group B and C were also smaller than those in group A and D( P< 0.05). Histopathological analysis shows that the area of CNV had close relationship with the retrocorneal membrane and the number of inflammatory cells( P< 0.05). No obvious side effect was round. Conclusion: Topical application of ATRA has a significant antiangiogenic effect against CNV indued by alkali burn.
