摘　　要：目的　探讨唐氏综合征产前筛查方法在中国人群中的运用 ,确定地域人群各孕周中位数。方法　2 0 0 1年 1月至 2 0 0 3年 3月采用全自动时间荧光免疫分辨技术 ,对广州市妇婴医院产前检查孕 9～ 2 0周的 75 0 1例妇女进行血清标志物———妊娠相关血浆蛋白 A(PAPP A)、甲胎蛋白 (AFP)、游离 β 人绒毛促性腺激素 (freeβ HCG)检测 ,并利用非线性加权回归计算中位数。结果　筛查出 6例唐氏综合征 ,1例假阴性。建立了地域性人群孕早、中期各孕周唐氏综合征筛查指标的中位数值 ,其孕周变化趋势与文献报道一致 ,用修改后的中位数重新分析 ,7例唐氏患儿全部检出。结论　孕妇血清筛查唐氏综合征的方法同样适用于中国人 ,人种对标志物有影响 ,各实验室应建立自己的中位数 。Objective To study the method of prenatal screening Down's syndrome in China,and to establish the median of each gestational week in the population of the region.Methods Auto time resolved fluoroimmunoassay system was used to detect the maternal serum markers in 7501 pregnant women at about 9～20 weeks of gestation-pregnancy associated plasma protein A(PAPP A)?alpha fetoprotein(AFP)?free beta human chorionic gonadotropin(free β HCG),and the median was calculated with weighted nonlinear regression.Results 6 cases of Down's syndrome had been screened out,1 trisomy 21 missed.The median of each gestation week in first and second trimester in the region had been set up and the changing tendency with gestational age was in accordance with the tendency reported by literatures.After recalculating with amending median,the 7 cases of Down's syndrome were found completely.Conclusion The strategy of maternal serum screening for Down's syndrome is applicable to Chinese people.The factor of race can influence the value of serum markers,so each laboratory should establish its own median in order to improve the effectiveness of screening.
