玻璃体后皮质剥离术在视网膜异物摘出中的价值

摘　　要：目的　玻璃体切除眼内异物摘出术中行玻璃体后皮质切除 ,观察其对预防术后远期并发症的作用。方法　 5 6例( 5 6眼 )常规玻璃体切除视网膜异物摘出术 ,术中行玻璃休后皮质剥离和切除 ,根据病情联合其他术式。术后随访观察视力及并发症发生率。结果　异物摘出成功率 10 0 % ( 5 6/ 5 6)。术后随访 3～ 12月 ,2眼 ( 3 6% )视网膜下异物术后 1月发生牵引性视网膜脱离 ,经再次手术 ,硅油填充 ,视网膜复位。术后矫正视力 0 5以上者 3 7眼 ( 66 0 7% )。结论　玻璃体后皮质剥离 ,有效去除外伤性PVR赖以生长的支架 ,解除玻璃体或机化条索对视网膜的牵引作用 ,可行效地预防外伤及术后视网膜脱离并发症的发生。Objective To investigate the value of posterior vitreous cortex (PVC)dissection in the extraction of retinal foreign body.Methods After pars plana vitrectomy and foreign body extraction,PVC dissection was performed routinely,other techniques were used according to different conditions.The post-operative vision and complications were followed up.Results Successful removal rate of the foreign bodies was 100%(56/56).Post-operation follow-up was 3-12 months.Tractive retinal detachment occurred at tine foreign body area in 2 cases of subretinal foreign body and the retina was reattached after a second surgery with silicone oil temponade.The corrected vision was better than 0.5 in 37eyes(66%).Conclusion Primary PVC dissection can effectively remove the scaffold for traumatic PVR, and relieve the traction of vitreous or organization strands to the retina so as to prevent and control the complications of traumatic retinal detachment.
