粤西地区消化性溃疡并发症及伴发病的胃镜诊断——附7700例分析

摘　　要：目的:探讨粤西地区 PU 并发症及伴发病(症)的胃镜诊断特点。方法:通过对我院17年(1981-1997)胃镜检出的 PU 7700例进行回顾性分析。结果:PU 主要并发症检出率19.53%;PU 并发出血522例,并幽门梗阻931例,并穿孔11例;687例 GU 并发出血45例(6.55%),并幽门梗阻25例(3.64%),并穿孔3例(0.43%);7013例 DU 并发出血517例(7.37%),并幽门梗阻906例(12.9%),明显高于 DU 并出血(P<0.05),并发穿孔8例(0.11%);7700例 PU 伴发病(症)5561例(72.22%),其中 PU 伴发胃炎为4565例(59.29%),687例GU 伴发胃炎196例(28.53%),7013例 DU 伴发胃炎4369例(62.30%),且以轻中度红斑性胃窦炎为主,P<005;PU 伴发其它疾病(症)996例(12.85%),以食道炎,球部炎症为主:5871中青年组(<60岁)PU 并发症655例(11.15%)低于老年组并发症(43.43%)(P<005);90年前3450例 PU 并发症540例(15.65%),低于90年后4250例 PU 并发症964例(22.58%)P<005。结论:粤西地区 PU 并发症检出率偏低,DU 并发幽门梗阻较高,PU 伴发胃炎以中度红斑性胃窦胃炎为主,萎缩性胃炎少见,PU 伴发其它疾病(症),以十二指肠球炎及食道炎多见,PU 并发症随年龄增高而上升,且近年亦有不断增高之势。To probe into the traits of gastroscopic diagnose of PU complications and Concomitant disease in Yuexi.Methods:7700 cases of PU examined by gastroscopy in 17 years(1981～1997)in our hospital were analysed retrospectively.Results:It was shown that the detectable rate of main complications of PU was 19.53%(1504 cases),of which 522 cases for complicated hemorrhage,931 cases for pylorochesis,11 cases for ulcerous penetrates.Of the 687 cases of GU,complicated hemorrhage was in 45 cases(6.55%),pylorochesis in 25 cases(3.64%),ulcerous penetrates in 3 cases(0.43%).Of the 7013 cases of DU,complicated hemorrhage was in 517 cases(7.37%),pylorochesis in 906 cases(12.9%),which is much higher than the complicated hemorrhage (P<0.05),and ulcerous penetrates in 8 cases(0.11%).Of the 7700 cases of PU,Concomitant disease was in 5561 cases(72.22%),of which 4565 cases(59.21%)were chronic gastritis.Of the 687 cases of GU,chronic gastritis was in 196 cases(28.53%);of the 7013 cases of DU,chronic gastritis was in 4369 cases(62.30%),which mainly were low-grade red macular gastritis and moderate(P<005).Other diseases were in 996 cases(12.85%), which were mainly duodenal bulbar duodenitis and esophagitis.Of 5871 cases of young and middle-aged group (<60),PU complications were in 655 cases(11.15%),which is lower than that of the old group(43.43%)(P<005). Before 1990,of 3450 cases of PU,complications were in 540 cases(15.65%),lower than 964 cases of 4280 cases (22.68%)after 1990.Conclusion:The detectable rate of PU complications in Yuexi is very low,and pylorochesis complicated by DU is higher than other complications,gastritis with PU is mainly moderate red macular gastritis, while the atrophic gastritis is rare.Among other Concomitant disease of Peptic ulcer,duodenal bulbar duodenitis and esophagitis are very common.The PU complications have relations with the age,and tend to rise in recent years.
