逐步递量加速超分割放射治疗食管癌32例

摘　　要：[目的]观察逐步递量加速超分割放射治疗(escalatedhyperfractionatedacceleratedradiationtherapy,EHART)食管癌的近期疗效和急性放射反应。[方法]将2002年～2003年64例病变长度<8cm的食管癌患者随机分为常规分割组(conventionalfractionradiationtherapy,CFRT)和EHART组,每组各32例。CFRT组采用2Gy/次、5次/周的方法,Dt=66Gy/33次/6～7周;EHART组在放射治疗的第1、2周,1.2Gy/2次/天熏两次间隔6小时以上,第3、4、5周分别为1.3、1.4、1.5Gy/2次/天熏均为5天/周,肿瘤灶总剂量66Gy/50次/5周。[结果]EHART组的急性放射反应明显较CFRT组严重(P<0.05),但均可耐受,全部顺利完成疗程。EHART组和CFRT组的原发灶近期疗效均比较好,完全缓解率(CR)分别为96.9%和93.8%。[结论]EHART能为大多数食管癌患者接受,且缩短了疗程,有较好的近期疗效。To assess the immediate responses and acute side-effect of escalated hyperfractionated accelerated radiation therapy for esophageal carcinoma.From April 2002 to March 2003,64 patients with esophageal carcinoma were randomly divided into two groups: conventional fraction radiation therapy(CFRT) groups and escalated hyperfractionated accelerated radiation therapy (EHART) groups,each group had 32 patients.Patients in CFRT group were received 20Gy per fraction, five fractionper week with the total tumor dose of 66Gy/33f/6~7w.Hyperfractionated irradiation was delivered with two fractions daily (with 6 hours interval) and five treatment days per week.In the first and second weeks,1.2Gy/fraction bid was given,then 1.3Gy/fraction bid ,in the 3rd week,1.4Gy/fraction bid,in the 4th week,and 1.5Gy/fraction bid in the 5th week.The total tumor dose was 66Gy/50f/5w.The acute side-effects were increased but acceptable in the EHART patients, and the radiation treatments were able to be completed without any interruption.The complete response rate (CR) was 96.9% in the EHART group and 93.8% in the CFRT group.[Conclusions]EHART could be tolerated by most of the patients with esophageal carcinoma with a better efficacy and shorten period of treatment.
