外科治疗对乳腺癌患者生活质量的影响

摘　　要：目的:观察不同外科手术对乳腺癌患者生活质量的影响,探讨选择更利于患者术后上肢功能恢复和提高生活质量的手术方式。方法:59例患者分为乳腺癌仿根治术组(29例)和乳腺癌根治术组(30例),分别根据患者自身基本条件和病理情况做乳腺癌仿根治术和乳腺癌根治术,并使用乳腺癌生活质量测评量表(FACT-B)对两组患者手术后进行随访和生活质量评估,在术后1～22d进行上肢运动训练。结果:乳腺癌仿根治术组与乳腺癌根治术组比较生活质量评分(≥41分)和上肢功能恢复(满意)差异均没有显著性意义(χ2=0.296,χ2=1.012,P>0.05),乳腺癌仿根治术组较乳腺癌根治术组两项评估均稍高。结论:乳腺癌仿根治术和乳腺癌根治术均对乳腺癌患者有良好疗效,配合上肢功能运动康复能得到满意效果。乳腺癌仿根治术组较乳腺癌根治术组患者生活质量、上肢功能满意程度稍高,可能与患者手术前发现时机有利于提早诊治有关。AIM:To observe the effects of different surgical operations on quality of life(QOL) in patients with mammary carcinoma,and to explore more advanced operational path for improving upper limb function and QOL of patients. METHODS:Fifty nine patients were divided into imitative radical operation of mastocarcinoma group(n=29)(imitative operation group) and radical operation of mastocarcinoma group(n=28)(operation group).According to their basic and pathological conditions,the patients were treated with radical operation of mastocarcinoma and imitative radical operation of mastocarcinoma respectively.Postoperative follow up and QOL were evaluated by using functional assessment of cancer therapy breast quality of life instrument(FACT B).One to twenty two days after operations,motive training of upper limb was taken. RESULTS:The scores of QOL(≥ 41) and the recovery of upper-limb functions(satisfaction) were not significantly different between the imitative operation group and the operation group(χ2=0.296,χ2=1.012,P >0.05).The assessments of the two items in the imitative operation group were better than those in the operation group. CONCLUSION:The imitative radical operation of mastocarcinoma and the radical operation of mastocarcinoma are both effective for patients.If combined with rehabilitation training,satisfactory effects will be obtained on motor function of upper limb.The more satisfactions on QOL and the motor functions of upper limb of the imitative operation group may be related to preoperative discovery,diagnosing and treating as early as possible and pathological conditions.
