摘  要:目的 观察后程加速超分割放射治疗非小细胞肺癌的近期疗效和急性放射反应。方法  16 5例非小细胞肺癌患者进入本研究 ,常规放射治疗 4 0 Gy后 ,改用后程加速超过分割推量治疗 ,1.5 Gy/次 ,2次 /d,5天 /周 ,总剂量至 70 Gy。结果  16 5例中 ,15 8例按计划完成治疗 ,近期疗效为完全缓解 13.9% (2 2 /15 8) ,部分缓解 5 9.0 %(10 9/15 8) ,总有效率 82 .9% (CR+PR)。放射性食管炎的发生率为 82 .3% (130 /15 8) ,放射性肺炎发生率为 2 3.4 %(37/15 8)。结论 后程加速超分割放射治疗非小细胞肺癌 ,近期疗效肯定 ,急性副反应绝大多数患者可以耐受 ,远期疗效和晚期并发症有待进一步的观察。Objective To assess the immediately responses and acute side-effects for non-small cell lung cancer (NSCLC) patients using late-course accelerated hyperfractionation radiotherapy.Methods From February 2002 to September 2003, 158 patients were treated with the conventional radiotherapy of 40Gy followed by late-course accelerated hyperfractionation radiotherapy. The late-course accelerated hyperfractionation radiotherapy was delivered with 1.5Gy per fraction, twice a day, 10 fractions a week. The total tumor dose was 70Gy. The immediately responses and acute side-effects were evaluated according to the WHO and RTOG criterion system. Results All patients completed their treatment course. The overall immediately response rate (CR+PR) was 82.9%. According to the RTOG criterion system, acute radiation esophagitis occurred in 82.3% of the total patients and acute radiation pneumonitis occurred in 23.4% of the total patients.Conclusion The late-course accelerated hyperfractionation radiotherapy can be considered as an effective and feasible approach to NSCLC with good responses and without any early serious complications. The long-term effect for survival and specific injury should be conducted for further study.
