摘 要:目的 探讨形态计量学在细胞学诊断中的有效的、参数指标。方法 应用自行研制的“分形分析系统 ,FAS” ,收集 5 3例经纤维内窥镜收集的人食管细胞学涂片 ,分别观察正常鳞状上皮细胞核、重度增生和食管癌细胞核的分形维数的改变情况。结果 发现正常食管上皮细胞核、重度增生和食管癌细胞核边界的分形维数均大于其拓扑维数 ,都有分形特征 ,分别为 1.0 42 6± 0 .0 167、1.163 8± 0 .0 2 11和 1.2 14 7± 0 .0 42 8,并且相互之间差异有非常显著性 (P <0 .0 1)。结论 分形维数可定量地反映细胞核形态的不规则程度 ,分形分析为肿瘤细胞的细胞学鉴别诊断提供了一种定量指标 ,具有一定的实际意义。Objective To find out a new reliable and sensitive concept in morphometry of tumor cytological diagnosis. Methods Cytological smears of 53 human esophagus were collected.'Fractal Analysis System, FAS' developed by ourselves was used to measure the Df of the nuclear boundary in HE-stained smears of esophagus which had been diagnosed. The normal squamous nuclei and severe atypical dysplasia of esophagus were tested, too. Results The Df of the nuclear boundary of normal esophageal squamous, severe atypical dysplasia and esophageal cancer were significantly higher than their topological dimension, so their structures were fractal. Their Df were 1.0427±0.0167, 1.1638±0.0211 and 1.2147±0.0428 respectively. There was highly significant difference between them as well ( P <0.01). Conclusion Fractal dimension can describe the irregularity of nuclear boundary quantitatively. There is practical values on fractal analysis theory in cytological diagnosis for esophageal cell.
