摘  要:目的:定量研究兔蛛网膜下腔出血(subarachnoidhemorrhageSAH)后海马CA1区微血管形态学的动态变化及相应的神经元病理改变,了解蛛网膜下腔出血后微循环障碍的特点。方法:采用枕大池两次注血法制作兔SAH模型,运用内源性过氧化物酶法显示兔SAH后3,5,7,10d海马CA1区微血管,用体视学方法对脑微血管进行定量分析,并观察了海马CA1区组织病理学变化。结果:SAH后3d海马CA1区微血管体积密度、长度密度、表面积密度分别为(401.69±58.49)mm/mm3,(4.39±0.56)mm/mm2,0.040±0.002,较正常组犤(448.28±47.30)mm/mm3,(5.41±0.38)mm/mm2,0.047±0.015犦均有明显下降,差异有显著性意义(P<0.05)。第7天最明显,且存在神经元的变性坏死;而血管直径在第7天才开始下降犤(3.93±0.86)μm比(4.89±0.83)μm,P<0.05)犦。结论:SAH后微循环障碍是继发性脑损伤的病理学基础,小血管的闭塞可能由于毛细血管灌注不良所致。AIM:To study quantitatively the morphological changes of microvessel in the CA 1 region of hippocampus after subarachnoid hemorrhage(SAH) and its corresponding pathological changes and to understand the characteristics of microcirculation disorder after SAH. METHODS:SAH models were produced by injecting autologous arterial blood into c isterna magna twice. The quantitative analysis of microvessel stained with intri nsic overoxidase in the CA1 region 3,5,7,10 days after SAH was performed using s tereological analysis system, and the pathological change was observed. RESULTS:Morphological parameters including volume, surface and length densitie s of the SAH group[(401.69±58.49) mm/mm3,(4.39±0.56) mm/mm2,0.040±0.002,respe ctively]decreased significantly as compared with those of the normal group[(448. 28±47.30) mm/mm3, (5.41±0.38) mm/mm2, 0.047±0.015 ](P < 0.05,especially at th e 7th day and associated with the degeneration of neuron ,whereas the diameter b egan to decrease at the 7th day[(3.93±0.86) μm vs (4.89±0.83)μm,P< 0.05]. CONCLUSION:Microcirculation disorder after SAH may morphologically contribute to delayed cerebral ischemia and occlusion of small vessels may be induced by ca pillary ill perfusion.
