眼内异物取出二次玻璃体手术分析

摘　　要：目的　探讨眼内异物取出第 1次玻璃体切除手术失败的原因 ,总结 2次玻璃体切除手术经验 ,提高 2次手术的成功率。方法　 9例 9只眼眼内异物 ,第 1次玻璃体手术后复发性视网膜脱离 ,经睫状体扁平部巩膜 3切口 2次玻璃体切除手术 ,切除晶状体及基底部残留玻璃体 ,视网膜切开松解 ,再次填充硅油及C3 F8。结果　 9只眼经 2次手术 ,随访 3～ 6个月 ,视网膜均复位。裸眼视力 :0 .0 4～ 0 .0 6 5只眼 ,0 .0 6～ 0 .0 84只眼 ,最佳矫正视力 0 .1～ 0 .15 6只眼 ,0 .2～ 0 .2 5 3只眼 ;眼压均在正常范围。结论　彻底切除前部增生性玻璃体视网膜病变(PVR)及巩膜穿刺内孔处玻璃体 ,周边部和瘢痕周围视网膜切开松解 ,防止术中及术后并发症是Objective To analyze the causes of failure to remove the intra-ocular foreign bodies in the first surgery and to summarize the experience of secondary vitrectomy.Methods Recurrent retinal detatchments in nine eyes of nine cases after the failure of first operation were treated with the secondary vitrectomy to remove the intra-ocular foreign bodies.The lens and peripheral residual vitreous bodies were removed.The retina was excised and refilled with silicon oil and C_3F_8.Results The 3～6 months of follow-up surveys showed that all eyes recovered after the secondary vitrectomy.The uncorrected visual acuity was 0.04～0.06 in 5 eyes,the corrected acuity was 0.1～0.15 in 6 eyes and 0.2～0.25 remained normal in all cases.Conclusion To resect anterior PVR and vitreous bodies near the Scleral hole,to excise the retinal scars and to relax retina are the key to successful secondary virtrectomy.
