摘  要:目的 :观察Li Pilocarpine大鼠癫痫模型在慢性期出现反复自发性癫痫发作过程中MRI与海马体积的进行性变化 ,探索癫痫发生的病理生理机制。方法 :分别在成功诱发癫痫持续状态大鼠的 3小时~ 8周的不同时间点分别行MRI成像与海马体积测量 ,并与正常对照组相比较 ,观察急性期至慢性期过程中的动态变化。结果 :在急性期最早累及杏仁核、内嗅皮层和梨状皮层 ,进而出现海马的水肿。两周后出现海马的进行性萎缩 ,海马体积在慢性期萎缩至正常的 1/2左右。结论 :癫痫持续状态后的脑损伤包括多个受累部位 ,既累及海马 ,也累及其它边缘系统 ,如杏仁核和邻近的梨状皮层、内嗅皮层、丘脑及大脑皮层感觉运动区。包括海马在内的边缘系统的改变更为复杂。Objective:This study investigates the progressive changes before appearing spontaneous recurrent seizures at the chronic period in the epilepsy model by MRI and volumetry of hippocampus and search for basic mechanisms of epileptogenesis.Methods:All the rats were monitored from 3 hours to 8 weeks after the onset of status epilepticus.MRI and volumetry of hippocampus were proformed at each time point and compared with the normal control groups to detect the changes from the acute to chronic period.Results:At acute period,the amygdala,the piriform and entorhinal cortex was firstly involved.Then edema could be found in the hippocampus. The atrophy of hippocampus began 2 weeks after status epilepticus,at the chronic period the hippocampus volume decreased to half of the normal rats.Conclusion:Multiple sites are involved after the status epilepticus.These sites not only included the hippocampus,but also the other limbic system such as the amygdala,the piriform and entorhinal cortex,the thalamus and the sensor-motor cortex are also involved.The changes of limbic system including hippocampus are more complex.
