摘 要:背景与目的:研究显示人类乳头状瘤病毒(humanpapillomavirus,HPV)的感染与口腔鳞癌的发生密切相关,但由于研究方法不同,结果差异较大,本研究采用Meta分析法综合评价口腔鳞癌的发生与人类乳头状瘤病毒感染的关系。方法:通过《中国生物医学文献数据库》收集1990年1月~2003年4月国内报道的有关口腔鳞癌发生与HPV感染关系的研究文献共44篇,其中符合研究标准的以聚合酶链反应(polymerasechainreaction,PCR)法检测的病例对照研究共10篇,采用Fisher法和Meta分析法对收集到的文献进行综合的定性和定量研究。结果:Fisher法定性分析显示,口腔鳞癌的发生与HPV感染存在相关关系(P<0.01);Meta定量分析显示,口腔鳞癌感染HPV或HPV16的合并优势比分别是正常口腔粘膜的8.89(3.62~21.80)和6.81(2.18~21.32)倍,口腔鳞癌HPV或HPV16平均检出率分别比正常口腔粘膜高49.02%(36.48%~61.57%)和45.74%(31.94%~59.54%)。结论:人类乳头状瘤病毒感染,尤其是HPV16,可增加口腔鳞癌发生的危险性。BACKGROUND &OBJECTIVE:Previous researches showed that human papi ll omavirus (HPV) infection was closely related to tumorigenesis of oral squamous c ell carcinoma (OSCC). But the results of these studies have great differences be cause of different research methods. This study was to evaluate synthetically th e relationship between OSCC of Chinese and HPV infection by meta analysis. METHO DS: From Jan. 1990 to Apr. 2003, 44 references reported about the relationship b etween tumorigenesis of OSCC of Chinese and HPV infection were collected from Ch inese Biomedical Literature Analysis and Retrieval System for Compact Disc (CBMd isc). There were 10 references accorded with research criteria which was case-c ontrol study, and detected by polymerase chain reaction (PCR). Fisher and meta analysis were used to quantitatively and qualitatively analyze these references synthetically. RESULTS: The synthetic qualitative analysis by Fisher showed the re was significant association between the tumorigenesis of OSCC and HPV infecti on (P< 0.005). The synthetic quantitative analysis by meta analysis revealed tha t the combined odds ratioes (ORc) for HPV, and HPV16 infection of OSCC were 8.89 (3.62-21.80), and 6.81 (2.18-21.32) times that of normal oral mucosa. The mea n positive rates of HPV, and HPV16 detection in OSCC were 49.02%(36.48%-61.57 %),and 45.74%(31.94%-59.54%) higher than that of normal oral mucosa. CONCLU SION: Human papillomavirus infection, especially HPV16, will increase the risk o f tumorigenesis of OSCC.
