放疗与VIP方案同步治疗Ⅲ期非小细胞肺癌的近期疗效观察

摘　　要：目的　评估放疗与VIP方案化疗同步治疗Ⅲ期非小细胞肺癌 (NSCLC)的近期疗效。方法　观察 1999年 12月至 2 0 0 2年 7月收治的 2 8例Ⅲ期非小细胞肺癌患者。予以放射治疗 ,放疗剂量为 5 0～ 70Gy/5～ 7周 ,同时行化疗 ,主要为VIP方案 :足叶乙甙 (VP -16) 75mg/m2 ,静脉滴注第 1～ 4天 ,异环磷酰胺 (IFO) 13 0 0mg/m2 ,静脉滴注 ,第 1～ 4天 (同时尿路保护 ) ;顺铂 (DDP) 2 0mg/m2 ,静脉滴注 ,第 1～ 4天 ,以上方案 2 1天为 1周期。结果　完全缓解率 (CR)为 2 1.4% ,部分缓解率 (PR)为 5 7.1% ,总有效率 (CR +PR)为 78.5 %。毒副反应主要为骨髓抑制、消化道反应、放射性肺炎及放射性食管炎。结论　放疗与VIP方案化疗同步治疗Ⅲ期非小细胞肺癌的近期疗效较为满意 ,毒副反应患者可以耐受。Objective To evaluate the short term response with radiotherapy plus concurrent etoposide-ifosfamide-cisplatin chemotherapy for stage Ⅲ non-small cell lung cancer.Methods From December 1999 to July 2002,28 non-small cell lung cancer patients entered the study.Radiotherapy:The radiation dose ranged from 50～70Gy/5～7weeks.Chemotherapy:The major regimen of chemotherapy was VIP,etoposide 75 mg/m 2,ifosfamide 1.3g/m 2(with uroprotection),and cisplatin 20 mg/m 2 given on days 1 to 4 of a 21-day cycle.Results The complete response rate was 21.4%,and partial response rate was 57.1%,the overall response rate was 78.5%.The major toxic effects were myelosuppression,gastrointerstinal tract reactions,acute radiation esophagitis and acute radiation pneumonitis.Conclusion The short term response with radiotherapy plus concurrent etoposide-ifosfamide-cisplatin chemotherapy for stage Ⅲ non-small cell lung cancer is preferable and tolerable toxicity.
