摘　　要：目的　探讨四肢恶性骨肿瘤综合疗法的临床疗效。方法　 2 3例恶性骨肿瘤患者化疗 2周后行肿瘤切除术 ,特制人工假体重建骨缺损。分析随访结果 ,包括术后疗效、生存情况、功能状况及处理。结果　 2 3例经过术后 3年 1个月～ 15年随访 (平均随访 6年 3个月 ) ,无瘤 5年生存率 34 8% (8/ 2 3) ,5年总生存率 5 2 6 %(12 / 2 3)。结论　术前全身化疗有利于杀灭血管内微小瘤栓 ,无瘤原则下切除肿瘤。Objective To investigate the clinical effect of combinated treatment of bone malignant tumors of extremities. Methods After systematical chemotherapy, 23 cases of bone malignant tumors were performed tumor resection and special-made prosthetic replacement to reconstruct the bone defect.The postoperation effect, survivorship, extremity function and management were analysed. Results 23 cases were followed up for 3 years and 1 month to 15 years, 6 years and 3 months on average. 34.8%(8/23) belonged to disease-free survival (DFS) at 5 years after surgery, and DFS probability at 5 years was 52.6%(12/23). Conclusion Minute tumor embolus in systemic circulation can be killed off by systematical chemotherapy before operation. It is emphasized that bone defect reconstructed by special-made prosthetic replacement should be based on the principle of non-tumors. Its an effective extremity salvage surgery treatment.
