摘  要:目的:探讨SPECT对急性缺血性脑血管病患者早期诊断的意义。方法:对同一患者在发病后24小时内进行SPECT及CT检查。结果:本组45例患者SPECT检查阳性检出率为91.1%(41?45),表现为病灶区rCBF减低或缺损,其异常部位与临床表现相符。CT异常检出率为15.6%(7?45),表现为低密度灶分布,其中6小时以内0例,6~24小时7例,短暂性脑缺血发作(TIA)0例,且SPECT所见病灶明显多于CT所见,SPECT与CT比较具有显著差异性(χ2=13.5,P<0.01)。结论:SPECT可探查到脑组织代谢和功能性损伤,可灵敏的探查到损伤部位的血流灌注变化,此时尚未发生形态学变化。SPECT对缺血性脑血管病的早期诊断及治疗有参考意义。Objective: To investigate the clinical value of 99mTc-ECD in the early diagnosis of patients with acute ischemic cerebrovascular disease. Method: Conducting SPECT and CT studies in the same patients less than 72 hours after the attack. Results: The positive rate of SPECT was 91.1%(4145). There was also a low regional cerebral blood flow in the affected area. The lesion was coincide with clinical symptoms. The positive rate of CT was 15.5%(745), the lesion appeared low density. After 6~24 hours and 24~72 hours from hypoxie-ischemic attack, 0, 7, 18 cases showed lesions on CT respectively. No case showed abnormality due to transient ischemic attack. The number of lesions showed on SPECT was more than that on CT. The difference was significant(χ2=13.5, P<0.01). Conclusions: 99mTc-ECD SPECT can easily demonstrate a metabolic and functional lesions without structural abnormalities. It can detect cerebral blood flow change of intracerebral foci not detectable by traditional morphological imaging, 99mTc-ECD SPECT may give some reference in the early diagnosis and therapy of patients with acute ischemic cerebrovascular disease.
