摘 要:近年研究证实胃幽门螺杆菌(Helicobacterpylori,HP)感染和许多常见上胃肠道疾病有关,但HP传染途径尚未明确。尽管流行病学资料支持Hp感染可能通过粪一口传播,但至今经大量试验尚未能从粪便中分离培养出HP,这提示十二指肠液(下称肠液)对HP有杀菌作用。我们进行体外和体内两组实验以探讨肠液对HP的影响。Studies in vitro and in vivo were perfor- med to investigate the effect of duodenal juice on Helicobacter pylori (Hp). In vitro study, fluid from the second part of the duodedum was collected from 65 patients. HP, H felts, Campylobacter jejuni or E colt were added to the duodenal aspirate. Viability was determin- ed after 30 minutes of incubation at room temporature, 90-96%of duodenal aspirates were bactericidal to HP and H. felts, but only 3- 6% to C jejuni and E colt (P<0.01) . In vitro study, gastric aspirates were collec- ted from 82 patients. Measurements of glyco- cholic acid levels and pH and the presence of HP in the stomach was analysed. Glycocholic acid levels in 46 patients with HP infection (39.0+63.4ug/ml) were significantly lower than those in 36 HP negative patients (100.2 +142.4ug/ml). We conclude that: (1) Bile is bactericidal to HP, both in vitro and in vivo. (2) HP is not the aetiologicai agent of bile reflux gastritis. (3) Bile may prevent the transit of HP through the intestine.
