膀胱灌注卡介苗加米托蒽醌预防膀胱癌术后复发

摘　　要：目的降低膀胱癌的术后复发率,提高膀胱癌的治疗效果。方法83例浅表性膀胱癌术后分为两组:①卡介苗加米托蒽醌组40例,卡介苗60mg加米托蒽醌10mg加生理盐水40ml,行膀胱灌注;②米托蒽醌组43例,米托蒽醌10mg加生理盐水40ml,行膀胱灌注。每周1次,连续8次,然后2周1次,连续8次,以后每月1次,总疗程2年。结果随访6～24个月,平均18个月。卡介苗加米托蒽醌组复发4例,复发率10.0%;米托蒽醌组复发16例,复发率36.4%。两组比较差异有显著性(P<0.05)。结论卡介苗加米托蒽醌膀胱灌注预防膀胱癌术后复发的疗效优于单用米托蒽醌。Objective To reduce the recurrence rate of the bladder cancer by the bladder installation of BCG plus mitoxantrone(MTZ).Method Eighty-three cases of superficial bladder cancer were divided into two groups after surgery:Group A,40cases treated with BCG plus MTZ.60mg BCG plus10mg MTZ plus40ml normal saline to be infused into the bladder;Group B,43cases treated merely with MTZ,being infused into the bladder at the dosage of10mg MTZ plus40ml normal saline,eight times in succession,once a week,and then eight times running,once every two weeks,and finally one in a month,with the total course of treatment being two years.Result Periods of follow-ups lasted6to24months(averaged18months).There were recurrences of four cases(10%)in the patients treated by BCG plus MTZ and sixteen cases(36.4%)of bladder cancer recurrence in the patients treated merely with MTZ.Therewas remarkable difference between two groups(P<0.05).Conclusion The curative effect in group A with BCG plus MTZ on prevention of the recurrence of superficial bladder cancer is superior to that in group B merely with MTZ.
