胃癌穿孔53例临床病理分析

摘　　要：回顾分析 1990年 1月 6日～ 2 0 0 3年 2月 18日入院治疗的 5 3例胃癌穿孔患者的临床病理资料。 40例( 75 5 % 40 / 5 3 )患者行根治性手术切除 ,死亡 4例 ( 10 % 4/ 40 ) ,中位生存期 3 4个月。 13例 ( 2 4 5 % 13 / 5 3 )行姑息性手术 ,死亡 8例 ( 61 5 % ) ,中位生存期 8个月。回顾分析的结果显示对胃癌穿孔的治疗应区别对待 ,积极争取行根治性手术或先做穿孔修补术 ,术后 3～ 4周再次手术切除肿瘤 。From June 1 1990 to February 18 2003,Fifty three cases of patients with gastric carcinoma with perforation were retrospective studied ,based on clinicopathological analysis.Forty patients were performed by radical gastroectomy,four patients(10%) were died,median survival was 34 months.Thirteen patients were managed involvoing with gastro-jejunal anastomosis,eight patients(61.5%) were died,median survival was eight months.The results of retrospective analysis show that treatment of gastric carcinoma with perforation should be carried out various methods depending on different cases,radical resection is performed as soon as possible,or the tumor are excised postoperative three to four weeks after initial simple closure of the perforation,if the patients were on bad condition,they should be also operated on simple closure of the perforation or then be done with bypass.
