羊膜移植治疗难治性浅表性角膜溃疡的临床观察

摘　　要：目的 :探讨羊膜移植在难治性浅表性角膜溃疡治疗中的可行性及治疗效果。方法 :对 32例经长期药物治疗效果不佳的难治性浅表性角膜溃疡患者采取病灶切除后羊膜覆盖术。结果 :2 7眼于术后 5 d～ 2周感染得到控制 ,溃疡愈合 ,角膜不同程度恢复透明。随访 3～ 12个月 ,2 7例患者原症状消失 ,角膜荧光素染色阴性 ,溃疡无复发。 5例分别于术后 1及 2个月复发 ,经羊膜移植联合板层角膜移植术后治愈 ,视力均有不同程度提高。结论Objective To find the availability and effect of the amniotic membrane transplantation in patients with refractory superficial ulcer of cornea.Methods Amniotic membrane transplantation was done to treat 32 patients with superficial corneal ulcer who had been treated with medicine for a long period but hadn't been cured. Results After 5 days to 2 weeks, inflammation was controlled in 27 eyes, and ulcers were disappeared. After 3 to 12 months, symptoms disappeared and ulcers didn't resume in 27 eyes. In the other 3 eyes ulcers resumed at 1 or 3 months after operations, then ulcers were cured by coneal transplantation. Visual acutes in different eyes were improved with different grades. Conclusion Amniotic membrane transplantation is an effective method to treat the refractory superficial corneal ulcer.
