摘 要:背景:常规数字减影血管造影(digitalsubtractionangiography,DSA)存在病变血管与周围血管重叠问题,应用旋转DSA(rotationaldigitalsubtrac-tionangiography,RDSA)及三维重建(three-dimensionalreconstructedim-agesbyuseofrotationaldigitalsubtractionangiography,3D-DSA)是否能解决这一问题?目的:评价RDSA及3D-DSA技术对脑血管疾病诊断的应用意义。设计:以诊断为依据的非随机对照研究。地点和对象:2002-05/2002-07解放军北京军区总医院收治的蛛网膜下腔出血患者8例,男6例,女2例,年龄28~70岁,平均43岁。干预:所有患者均应用传统DSA,RDSA及3D-DSA技术进行检查。血管造影机使用GE公司AdventxLCV数字减影血管造影系统及MarkVProvis高压注射器。用于图像后处理的计算机为采用SunSpareUNIX系统的AW4.0三维处理工作站。图像的采集和处理通过图像存储和传输系统(picturearchiveandcommunicationsystem,PACS)连接。主要观察指标:不同技术方法获得的血管造影图像质量、颅内病变血管位置及病变结构显示情况。结果:所有患者均确诊为颅内动脉瘤。常规正侧位DSA能显示存在动脉瘤病变或可疑存在动脉瘤,仅极少数病例能明确显示瘤颈形态(2/8)和载瘤动脉与瘤体的关系(1/8)。RDSA显示病变复杂结构的能力较常规DSA明显提?BACKGROUND:There is overlapping problem existing between focal blood vessels and peripheral blood vessels when using routine digital subtraction angiography(DAS).If using rotational digital subtraction angiography(RDSA) and three-dimensional reconstructed images by use of rotational digital subtraction angiography(3D-DSA) can resolve this problem is still uncertain.OBJECTIVE:To evaluate the application of RDSA and 3D-DSA techniques to diagnosis of cerebrovascular diseases.DESIGN:Non-randomized case control study on the basis of diagnosis.SETTING and PARTICIPANTS:Eight patients with subarachnoid hemorrhage treated in General Military Hospital during May 2002 to July 2002 were selected with 6 males and 2 females,aged from 28 to 70 years old with mean age of 43 years old.INTERVENTIONS:All the patients have taken the traditional DSA,RDSA and 3D-DSA tests.Angiographic machine was chosen the Adventx LCV digital subtraction angiographic system produced by GE Company and Mark V Provis high pressure injector.The computer used to process the image was AW4.0 three dimensional management workstation adopted Sun Spare UNIX system.The gathering and process of imaging was connected by picture archive and communication system(PACS). MAIN OUTCOME MEASURES:Image quality of angiography by different techniques,location of affected blood vessels and structure of affection.RESULTS:All the patients were diagnosed with intracranial aneurysm. Regular front position and lateral position DSA could present the existence of aneurysm or suspicious aneurysm, but only in very few patients it could clearly display the neck shape(2/8) as well as the relationship between carrying artery and the body of aneurysm(1/8).RDSA was good at presenting the complicated structure of aneurysm when comparing with regular DSA.3D-DSA images of 8 patients clearly showed the body,neck shape and carrying artery of aneurysm.CONCLUSION:RDSA combining 3D-DSA technique is very good on showing the pathological changes of intracranial aneurysm.It is helpful to the diagno
