摘  要:目的 :探讨具有促进淋巴管内皮细胞增殖和毛细淋巴管增生的淋巴管内皮生长因子 C(VEGF C)在膀胱移行细胞癌 (BTCC)中的表达及其与肿瘤淋巴管浸润、淋巴结转移等临床病理因素的关系。方法 :采用免疫组织化学方法研究 4 5例BTCC中VEGF C的表达水平 ,观察评估VEGF C的阳性表达与淋巴管浸润、淋巴结转移等临床病理资料的相关性。结果 :4 5例中 ,VEGF C阳性 36例 ,VEGF C的阳性表达与肿瘤分级、分期、静脉或淋巴管浸润、淋巴结及前列腺侵犯明显相关 (P <0 .0 5 ) ;多因素分析显示VEGF C的表达是唯一影响盆腔淋巴结转移的独立因素 (P =0 .0 0 3)。结论 :VEGF C与BTCC淋巴管浸润和转移有显著相关性 ,检测BTCC中VEGF C的表达能够预示盆腔淋巴结是否转移。Objective:Vascular endothelial growth factor C (VEGF-C) was demonstrated to stimulate the growth of lymphatic vascular endothelium. The purpose of this study is to determine whether VEGF-C correlates with lymph node metastasis in Bladder Transitional Cell Carcinoma(BTCC).Methods:We assessed VEGF-C expression using immunohistochemistry in 45 Bladder Transitional Cell Carcinomas with long-term follow-up. The relationship among VEGF-C expression, lymph node status, and other established clinicopathological parameters was assessed.Results:VEGF-C expression was identified in 36 cases(80%) with immunohistochemistry. Positive VEGF-C was significantly correlated with poorer ologic grade,depth of invision,lymphatic and venous invasion, lymph node metastasis and prostate metastasis(P< 0.05 ). Multivariate analysis revealed VEGF-C expression to be the sole independent factor influencing pelvic lymph node metastasis(P= 0.03 ).Conclusions:VEGF-C expression may be associated with disease outcome through the promotion of lymphatic involvement/metastases in BTCC. Examination of VEGF-C expression in specimens may be beneficial in the prediction of pelvic lymph node metastasis of BTCC.
