PCNA,Ki-67对胃肠道间质瘤预后的临床病理学探讨

摘　　要：【目的】探讨PCNA ,Ki 6 7在胃肠道间质瘤 (GIST)中的不同表达 ,为GIST的临床治疗和预后判断提供新的病理学依据。【方法】对 4 1例非上皮源性肿瘤患者 ,用光镜和CD1 1 7筛选 ,胃肠道间质瘤为 32例 ,其中2 7例复染PCNA ,Ki 6 7,并利用病理图象分析系统进行资料分析 ,最后用统计学进行比较。【结果】CD1 1 7阳性87 5 %。PCNA增殖指数良性组低于恶性组 ,与转移组无显著差异 ,而且增殖指数 >2 0时恶性几率明显增高 ;Ki 6 7增殖指数远低于恶性组和转移组 ,且增殖指数 >1 0时 ,恶性几率明显增高。【结论】PCNA ,Ki 6 7的增殖指数增高 ,恶性几率增高 ,而且Ki 6 7与肿瘤转移有关。因此 ,在良恶性不甚明确的肿瘤中 ,二者可联合或单独作为预后的参考指标。To study the different expression of PCNA,Ki 67 in gasttrointestive stromal tumors,and provide a new pathological basis for clinnicians in determination of further treatment and prognosis.forty one gastrointestinal mesenchymal tumors were retrospectively studied with microscope and CD117.Thirty two cases were GIST.Then we select twenty seven cases and analyse the result of PCNA and Ki 67 with the pathology photogram analysis system and statistic.The positive rates of CD117 were 87 5%.The proliferation rate(PR) of PCNA:the benign groupwas lower than the malignant.But showing no statistical difference between denign group and metabasis group.When PR>20,the chance of malignant was higher.But the PR of Ki 67:the benign group was much lower than the malignant and metabasis group.And when PR>10,the chance of malignant was higher.[Conclusion]The PR of PCNA and Ki 67 was higher,and the chance of malignant was higher.The PR of Ki 67 was associated with the metabasis of tumors.So assosiation or division of PCNA and Ki 67 are reference parameters for the prognosis of indifinite characteristic GIST.
