单药赫赛汀~治疗复发转移性乳腺癌

摘　　要：目的　评价单药赫赛汀 治疗HER2过表达的转移性乳腺癌的疗效及不良反应。方法应用赫赛汀 单药治疗复发转移性乳腺癌患者 2 2例 ,采用两种给药方案 :(1)每周方案 :第 1周给予负荷剂量 4mg/kg ,以后每周给予 2mg/kg ;(2 ) 3周方案 :每 3周给药 1次 ,第 1次给予负荷剂量 8mg/kg,以后每 3周给药 6mg/kg。结果　 2 0例可评价疗效。其中无完全缓解 (CR)病例 ,部分缓解 (PR) 5例(2 5 .0 % ) ,病情稳定 (SD) 5例 (2 5 .0 % ) ,病情进展 (PD) 10例 (5 0 .0 % ) ,总有效率为 2 5 %。按治疗意向人群 (ITT) 2 2例患者计算 ,总有效率为 2 2 .7%。中位疾病进展时间是 6周 ,中位治疗失败时间是6 .5周。最常见不良反应是发热和寒战 ,少数患者有心脏不适。结论　赫赛汀 是治疗HER2过表达的转移性乳腺癌的有效药物 ,不良反应可耐受。Objective To evaluate the efficacy and adverse reactions of Hercept in as a single agent in patients with HER2 overexpressing metastatic breast ca ncer. Methods There were two kinds of therapy protocol. One was loading-dose of 4 mg/kg intravenously,followed by a 2 mg/kg maintenance dose of weekly interva ls . The other was loading-dose of 8 mg/kg,followed by 6 mg/kg of every three wee ks intervals. Results Among 20 patients with evaluated efficacy,there was no com plete response,5 patients (25.0%) showed partial response (PR),5 (25.0%) stabl e disease (SD) and 10 (50.0%) progressive disease (PD). Of 22 patients,the over all response rate was 22.7%. The median time of disease progression and treatmen t failure was 6 weeks and 6.5 weeks,respectively. The most common adverse react ions were fever and chill. Cardiac symptoms could be seen in some patients. Conclusion Herceptin is an active agent for the patients with HER2 overexpressing metastatic breast cancer and the adverse events are well tolerated.
