摘 要:目的:探讨健康教育对恢复期住院精神分裂症患者的康复作用。方法:将2003-1/2003-12在济宁医学院第二附属医院住院并且符合中国精神障碍分类与诊断标准第3版关于精神分裂症诊断标准,达到恢复期的250例患者随机分成两组,对其中的125例进行集体健康教育(干预组),另125例为对照组(未干预)。采用简明精神病量表(BPRS)及自制一般资料调查表,由3名研究人员分别在干预前和干预后3个月时对患者BPRS总分、服药依从性、疗效及攻击行为变化进行评定。结果:干预组在干预后3个月时患者的BPRS总分(21.87±4.45)较干预前(26.21±4.96)降低,差异有极显著性意义(t=6.48,P<0.001);服药依从性(完全依从93例,部分依从28例)较对照组(完全依从63例,部分依从37例)好,差异有非常显著性意义(t=3.25,P<0.01);疗效(痊愈+显著进步86例)好于对照组(痊愈+显著进步58例),差异有非常显著性意义(t=3.63,P<0.01);攻击行为(45.96±9.13)较对照组(49.93±9.26)降低,差异有非常显著性意义(t=4.65,P<0.01)。结论:健康教育对恢复期住院精神分裂症患者,能够提高服药依从性及疗效,减少攻击行为的发生,提高患者的生活质量。AIM:To investigate the effects of the health education on the recovery of the inpatients with schizophrenia in convalescence. METHODS:A total of 250 inpatients with schizophrenia,who were in the Second Affiliated Hospital of Jining Medical College during January to December 2003 and met the diagnostic criteria of the third edition of[Chinese Classification and Diagnostic Criteria of Mental Disease](CCMD 3),were randomly divided into intervention group(n=125,receiving health education) and control group(n=125,receiving no intervention).The total score of brief psychiatric rating scale(BPRS),compliance,curative effect and the change of aggressive behavior were evaluated with the BPRS and self designed questionnaires about the general information by 3 investigators before and 3 months after intervention. RESULTS:The total scores of BPRS in the intervention group 3 months after intervention(21.87±4.45) was extremely significantly decreased as compared with that before intervention(26.21±4.96)(t=6.48,P< 0.001). The compliance in the intervention group(total compliance in 93 cases and part compliance in 28 cases) was better than that in the control group(total compliance in 63 cases and part compliance in 37 cases),and the difference was very significant(t=3.25,P< 0.01).The curative effect in the intervention group(86 cases were fully cured or significantly improved) was better than that in the control group(58 cases were fully cured or significantly improved),and there was very significant difference(t=3.63,P< 0.01).The aggressive behavior in the intervention group(45.96±9.13) was reduced as compared with that in the control group(49.93±9.26),and there was very significant difference(t=4.65,P< 0.01). CONCLUSION:Health education can raise the compliance and curative effect,reduce the occurrence of aggressive behavior,improve the quality of life and the quality of medical nursing care.
