摘　　要：目的　探讨经远外侧枕下入路切除高颈段椎管内外哑铃型肿瘤及下斜坡肿瘤的治疗效果及手术技巧。方法　对 14例高颈段椎管内外哑铃型肿瘤及下斜坡肿瘤 ,采用外侧枕下入路进行手术切除。结果　肿瘤全切除 10例 ,次全切除 4例。所有患者术后恢复良好 ,未出现与手术入路有关的严重并发症。结论　远外侧枕下入路具有手术径路短、视野宽广、显露硬膜内外病变清楚等优点 ,可减少不必要的手术步骤 ,是治疗高颈段椎管内外哑铃型肿瘤及下斜坡肿瘤的一种有效手术方法。Objective To explore the operative result of far lateral suboccipital approach to remove dumbbell tumors of the upper cervical spinal canal and tumors of inferior clivus and to find some operative techniques.Methods The far lateral suboccipital approach was used in 14 patients with tumors of the upper cervical spinal canal and inferior clivus.Result Tumors were totally removed in 10 and partially totally removed in 4 patient.All patients got good recovery.No complications due to operations occurred.Conclusion The far laterial suboccipital approach is characterized by short approach and wide operation field.It is an effective approach to remove the dumbbell tumors of the upper cervical spinal canal and the tumors of inferior clivus.
