持续性VCG监护技术在胃镜检查中的应用

摘　　要：①目的　将一种造价低、性能好、操作简便、适合胃镜检查的持续性心电向量图 (VCG)缺血监护设备 ,试用于胃镜检查者 ,评价其作用与效果。②方法　用现有临床上常用的心电图机、通用计算机和图形功能较好的激光打印机等组成硬件结构 ;采用改良的IESA导联连接方式获取正交心电图信号 ,利用软件实现DOWER矩阵方程换算 1 2导联同步心电图。用该仪器对 1 70例消化内镜受检者进行持续性VCG监护。③结果　胃镜术中 90 .5 5 %的病人出现心率增快 ,出现的心律失常以窦性心动过速、室性期前收缩及房性期前收缩多见 ,ST T改变以T波低平及ST段压低为主。④结论　非特异ST T段异常特别是缺血型的ST T异常检出阳性率明显高于单道监护 ,持续性VCG监护技术在胃镜检查中具有较重要的实用意义。Objective To design a vectorcardiography(VCG) monitoring apparatus at low cost,with good quality,and easy operation for gastroscopy, and to evaluate its effects in the procedure. Methods The hardware consisted of common ECG machine, general computer and laser printer with good graphic function. Modified IESA lead connection was adopted to obtain orthogonal ECG signals and to achieve DOWER matrix equation by a software which was transformed to 12 lead synchronism ECG. It was used in 170 patients undergoing gastroscopy for continuous monitoring. Results The heart rate of 90.55% of the patients increased during the procedure, which included sinus tachycardia, ventricular premature beat or atrial premature beat. The main changes of ST-T segment were T wave flattening or inversion or ST segment depression. Conclusion The detection rate of non-specific ST-T segment changes ,especially ischemic ST-T segment change, by VCG is obviously higher than by using single channel monitoring system. Persistent VCG monitoring is of practical significance in gastroscopy.
