摘 要:目的 :探讨黏蛋白 1(MUC1)和midkine(MK)在外阴上皮内瘤变 (VIN)和外阴鳞状细胞癌 (VSCC)中的表达及临床意义。方法 :采用免疫组化方法对 5 0例VSCC ,2 5例VIN ,10例正常外阴表皮进行研究 ,同时将MUC1和MK表达与外阴肿瘤的临床病理特征相比较。结果 :VSCC中MUC1蛋白表达及阳性细胞率高于VINⅢ (P <0 .0 5 ) ,VINⅠ ,Ⅱ和正常表皮无表达 ;MUC1阳性细胞率在高、中、低分化VSCC中差异显著 (P <0 .0 5 )。VSCC中MK蛋白表达及阳性细胞率高于VINⅢ (P <0 .0 5 ) ,VINⅠ ,Ⅱ和正常表皮中无表达 ;MK更强表达于分化型 (高分化和中分化 )的VSCC中 ,与未分化 (低分化 )的VSCC相比差异显著 (P <0 .0 5 )。结论 :MUC1表达可作为VSCC一种有效的肿瘤标志物。MUC1和MK与外阴肿瘤细胞分化有关 ,随着细胞分化程度的降低 ,MUC1表达增加 ,MK表达减弱。Objective: To clarify the role of mucin 1 (M VC1) and midkine(MK) in vulvar carcinogenesis by determining the expressions in vulvar intraepithelial neoplasia (VIN) and vulvar squamous cell carcinoma (VSCC) . Methods: Fifty cases of VSCC and 25 of VIN were included i n this study. The streptavidin-biotin-immunoperoxidase method was used. Expressi ons of MUC1 and MK was compared with clinicalpathologic features of vulvar tumor s. Results: The expression of MUC1 in the VSCC cases was hig her than that in cases of VIN stage Ⅲ.The difference between VSCC and VINⅢ was statistically significant (P<0.05). No expression was detected in the VIN stage I to II and normal epithelium. The percentage of MUC1 positive cells were significantly different in well, moderately, and poorly differentiated VSCC(P <0.05). The expression of MK and percentage of positive cells in VSCC cases we re significantly higher than those in cases of VIN stage Ⅲ (P<0.05). MK exp ression was undetectable in VIN stage I to II and normal epithelium. MK was more intensely expressed in differentiated (well and moderately differentiated) VSCC than undifferentiated (poorly differentiated) VSCC (P<0.05). Concl usion: MUC1 expression may be an effective tumor marker for VSCC. The appearance of MUC1 and MK are linked to cell differentiation. MUC1 expression b ecame gradually stronger, while MK expression became weaker as the degree of cel l differentiation lowered.
