仅存光感的严重眼外伤手术疗效分析

摘　　要：目的　分析 2 0例 (2 0眼 )无光感或光定位不确的眼外伤施行显微手术的疗效及影响疗效的因素。方法　严重眼外伤入院后常规眼科检查及辅助检查包括B超扫描、X线断层扫描及视觉电生理检查。施行Ⅱ期玻璃体视网膜手术 ,摘出眼内异物。结果　术后视网膜在位 16眼 ,后极部视网膜在位 1眼。术前无光感 3眼中术后恢复光感者 1眼。术前光定位不确17眼中恢复数指以上视力者 10眼 ,其中 0 0 5～ 0 3者 6眼。结论　仅存光感的严重眼外伤视功能丧失主要原因包括视神经、视网膜、脉络膜严重广泛受损和毁坏。视 枕诱发电位波形消失者 ,视力可能完全丧失。波形波幅严重下降者 ,经玻璃体视网膜手术有可能恢复不同程度的视力。Objective To evaluate treatment effect and influential factors in the no light or only light perception traumatic eye.Methods Retrospective analysis of the clinical data and results.General ophthalmic examination,B-Scan,CT,F-VEP and ERG were performed in 20 eyes with serious truma.Retina-vitreous operation was treated after a few days or a few weeks when the wound was closed. Results In 16 eyes,the retina was all reattachment and another one was reattachment of posterior fundus area. One eye was recovered light perception in 3 eyes with no light perception.10 eyes were recovered various degree visual acuity from counting finger to 0.3 respectively.Conclusion The most important causes of losing visual function are defective or destructive injuries of optic rterve,retina and choroid. The retina-vitreous operation will be successful when F-VEP wave and ERG is not negative,and recovered different degree visual acuity.
