瞳孔成形术在眼外伤修复术中的作用

摘　　要：目的　探讨瞳孔成形术在眼外伤修复术中的作用。方法　采用多种瞳孔成形技术联合其他眼科手术 (如角膜修补术、人工晶状体植入术及玻璃体切除术等 )治疗各种伴虹膜损伤的眼外伤 43眼。随访 3月～ 2年。结果　 42眼术后视力较术前有不同程度的提高 (z =-5 5 67,P =0 0 0 0 ) ,无一眼出现单眼复视。所有病例术后瞳孔呈圆形或类圆形 ,瞳孔中央位 40眼 ,瞳孔偏位 3眼。术前伴继发青光眼 5眼 ,术后眼压控制。术后前房积血 3眼 ,玻璃体积血 1眼 ,经保守治疗痊愈。人工晶状体光学部边缘暴露于瞳孔区 1眼。结论　瞳孔成形术既能恢复虹膜的完整性 ,又能改善光学效果和美容效果 ,还能稳定眼内环境 ,在眼外伤修复中有重要作用。Objective To explore the effect of coreoplasty on ocular injury.Methods The coreoplasty associated with other ocular operations(corneal surgery,intraocular lens (IOL) implantation,vitrectomy,)was performed on ocular injury with iris trauma.Follow-up ranged from 3 months to 2 years.Results The postopterative visual acuity increased to varying degrees in 42 of 43 eyes.The symptoms of diplopia in five patients were disappeared after operation.The pupil of all patients were round or near round.The pupil in center was in 40 eyes,and deviated in 3 eyes.The postoperative intraocular pressures were controlled in 5 patients with secondary glaucoma.Hyphema in 3 eye and vitreous hemorrhage in 1 eye were recovered by conservative treatment.The edge of IOL exposed in the pupillary area occurred in 1 eye.Conclusion The coreoplasty not only recovered integrity of iris,but also improved photolog and cosmesis,and stabilized the ocular internal environment.The coreoplasty possessed important action on ocular injury repair.
