摘  要:目的 观察短暂性脑缺血发作 (transientischemicattacks ,TIA)对脑梗死患者的神经保护作用。方法 选择 2 0例首次发作脑梗死的患者作为脑梗死组 ;有同侧TIA并且为首次发作脑梗死的患者 2 0例作为TIA组。另外选择 2 0例行下肢手术的患者作为对照组 ;脑梗死组与TIA组患者入院后均行颅脑CT确诊。应用酶联免疫吸附试验和放射免疫法分别检测三组患者脑脊液(CSF)中神经元特异性烯醇化酶 (neuron specificenolase ,NSE)和S 10 0含量。同时测定脑梗死组和TIA组患者脑梗死容积。 结果 脑梗死组和TIA组CSF中NSE和S 10 0水平高于对照组 (P <0 .0 1) ,而TIA组又低于脑梗死组 (P <0 .0 1)。TIA组患者的梗死容积小于脑梗死组 (P <0 .0 5 )。脑梗死组和TIA组CSF中NSE和S 10 0含量与梗死容积的大小呈明显正相关 (P <0 .0 1)。结论 TIA可降低首次发作脑梗死患者CSF中NSE和S 10 0的释放 ,减少脑梗死容积 。Objective To evaluate the effect of transient ischemic attack (TIA) on cerebral infarction volume and the production of neuron specific enolase (NSE) and S 100 of patients with cerebral infarction.Methods Sixty patients including 20 patients with first ever cerebral infarction (Group CI), 20 first ever cerebral infarction patients with prior ipsilateral TIA (Group TIA) and 20 control subjects (Group control) were involved in study. The concentrations of NSE and S 100 in cerebrospinal fluid(CSF)in three groups were measured with enzyme linked immunosorbent assay (ELISA) and radioimmunoassay respectively. The cerebral infarction volume of patients in Group CI and Group TIA were evaluated. Results The levels of NSE and S 100 in CSF were increased in Group CI comparing with Group control (P<0.01), and these were decreased in Group TIA comparing with Group CI (P<0.05). The infarction volume was smaller in Group TIA than in Group CI (P<0.01). The concentrations of NSE and S 100 in CSF of TIA in Group CI and Group were positively correlated with infarction volume (P<0.01).Conclusion TIA could decreased the secretion of NSE and S 100 and the infarction volume of patients with first ever CI. And it may have a neuroprotective effect on cerebral infarction.
