皮质电图监测联合术式治疗难治性癫痫

摘　　要：目的　对难治性癫痫行几种手术方式结合治疗 ,以期达到良好的疗效。方法　 1 8例难治性癫痫。术前行常规EEG ,AEEG ,CT ,SPECT检查者 1 8例 ;MRI检查者 4例 ;PET检查者 1例 ;曾行γ刀治疗 1例。所有病例术前均行智商测定 ,综合各项检查 ,判癫痫灶可能存在的部位 ,应用联合术式行手术治疗。结果　术后随访 1～ 7年 ,完全不发作者 1 1例 ;发作显著改善者 5例 ;发作轻微改善者 2例。结论　多种术式联合治疗难治性癫痫安全有效。Objective The investigate effect of combined operations for refractory epilepsy. Methods 18 patients with refractory epilepsy were examined by electroencephalogram (EEG), active EEG, CT and single photon emission computed tomography (SPECT). Among them, 4 patients were examined by magnetic resonance imaging (MRI); one patient was examined by positron emission tomography (PET); one patient was received radiotherapy of γ-ray. Everyone's intelligence quotient was examined before operation. The focus of epilepsy was confirmed by these examinations. Combined operation was performed for complex intractable. Results all patients were follow up from 1 to 7 years. 14 patients had not epileptic attack; epileptic attack was ameliorated obviously in 5 patients and slightly in 2 patients. Conclusions Combined operation is an effective and safe method for refractory epilepsy.
