国产力蒙欣与咪唑安定用于胃镜检查术的临床观察

摘　　要：目的 :研究 3种不同给药方法在纤维电子胃镜中的镇静作用及对循环呼吸的影响。方法 :选择自愿接受无痛胃镜的患者90例 (按美国麻醉医师学会对术前病人的病情估计分级法ASAⅠ ～ Ⅱ级 ) ,随机分为 3组 :Ⅰ 组 (力蒙欣 2mg/kg) ,Ⅱ 组 (咪唑安定 0 .0 8mg/kg) ,Ⅲ 组 (力蒙欣 1 .3mg/kg+咪唑安定 0 .0 4mg/kg) ,每组各 30例。观察术中HR ,MAP ,SPO2 及镇静分级、苏醒时间、记忆缺失、呼吸抑制及自我感觉情况。结果 :Ⅰ 组MAP ,SPO2 下降 (注药后 1～ 3min)与 Ⅱ、Ⅲ两组比较差异有显著性 (P <0 .0 5) ;Ⅲ 组MAP(注药后 1～ 3min)虽有下降趋势 ,但与 Ⅱ 组比较差异无显著性 (P >0 .0 5)。镇静分级、苏醒时间、记忆缺失、呼吸抑制及自我感觉 Ⅰ、Ⅲ 两组组间无差异 ,但均明显优于 Ⅱ 组 (P <0 .0 5)。结论 :力蒙欣 1 .3mg/kg ,复合咪唑安定 0 .0Objective:To study the function of sedation and effect on circulation and breathing via three different administration of drug during operation of fiber gastroscope.Methods:Chose the volunteer who accept no-pain gastroscope,randomed into three group.Group Ⅰ were administrated propofol 2 mg/kg,group Ⅱ Imidazolem ansiolin midazolam 0.08 mg/kg,group Ⅲ propofol 1.3 mg/kg + midazolam 0.04 mg/kg, each have 30 cases.HR,MAP,SPO 2,classification of sedation,awake time,memory deficit,and self-sensation on the operation were obsrved.Results:After 3 min administration of drug, the decrease of MAP, SPO 2 have significant difference among group Ⅰ and Ⅱ,Ⅲ(P<0.05).Although MAP(after 3 min)at group Ⅲ tend to decrease,compared with group Ⅱ,there was no significant difference(P>0.05).About classification of sedation ,awake time,memory deficit,and self-sensation,there were no difference among group Ⅰand group Ⅲ,but the two group is obviously better than group Ⅱ(P<0.05).Conclusion:Propofol 1.3 mg/kg,combined with midazolam 0.04 mg/kg were given on the operation of gastroscope,which is safe and effective.
