双蒂胸骨舌骨肌筋膜瓣在喉功能重建中的应用

摘　　要：目的　探讨胸骨舌骨肌及甲状软骨膜制作双蒂软骨膜瓣修复喉部分及大部分切除术的疗效。方法　垂直部分喉或扩大垂直部分喉切除术或喉次全切除术。结果　 6 7例中于术后第 8～ 1 7天内均能拔除气管套管 ,拔管率为 1 0 0 %。喉次全切除术 1 5例 ,气管套管保留时间较长 ,术后 1 5～ 5 7天内拔除气管套管后 ,吞咽、呼吸功能恢复良好 ,但声音有不同程度的嘶哑。 82例手术五年生存率 72 %。结论　依据喉解剖学结构设计手术方法 ,修复喉腔 ,既能切除喉癌组织 ,又能够保留喉的功能 。Objective To explore the curative effect of making bi-pedical perichondrium flaps use of sternohyoid muscle and thyroid cartilage periosteum to repair the defeet after partial and most partial largngectomy for 82 cases.Methods To use the laryngectomy. Results In the 67 cases, the patients all can be extracted after 8-17 days.The tracheal tubes extraetion rate was 100%. To hemilaryngeetomy 15 cases,the remaining time of tracheal tubes need a little longer, after 15-75 days, tubes were extracted, the function of swallow and breathe recovered very well, but the sound is hoarse in defferent level. The 5-year surgical survival rate for 82 cases is 72%. Conclusion According to the surgical method of laryneal cavity,not only can excise the function the largnx, but also can get the satisfied effect.
