外伤性白内障摘除与人工晶状体植入临床报告

摘　　要：目的:报告外伤性白内障摘除与人工晶状体植入的临床研究情况。方法:回顾性研究外伤性白内障手术治疗患者633例,植入人工晶状体者455例(459眼)。观察术前术后并发症和视力情况。结果:术后并发症主要有葡萄膜炎症反应(49.8%),角膜水肿(43.0%),后囊混浊(36.2%)等。术后视力增加者526眼(79.7%),视力减退者15眼(2.3%),视力无改善者47眼(7.1%);仍有145眼处于单眼盲水平,约占22.0%;而低视力者137眼(20.8%);达到0.3以上有用视力者306眼(46.4%)。结论:外伤性白内障患者以青壮年和少年儿童为多,病情较为复杂,并发症多,手术难度大,其手术成功与否和视力预后取决于受伤眼的损伤程度、并发症情况和手术医生的技术与设备条件以及手术时机等因素。AIM: To report the clinical study on traumatic cataract extraction and intrao cular Lens (IOL) implantation.· METHODS: A total of 635 cases who underwent traumatic cataract operation were studied retrospectively, among whom 455 cases (459 eyes) were performed IOL imp lantation. Preoperative and postoperative complications and visual acuity were o bserved.· RESULTS: The common postoperative complications induced uveal reaction (49.8% ), corneal edema (43.0%) and posterior capsule opacity (36.2%). Postoperative vi sual acuity was improved in 526 eyes (79.7%), unimproved in 47 eyes (7.1%) and b ecame worse in 15 eyes (2.3%). Still 145 cases (22.0%) remained unilateral blind ness and 137 eyes (20.8%) obtained low vision, and 306 eyes (46.4%) got visual a cuity of 0.3 or better.· CONCLUSION: Most cases of traumatic cataract were youth and children. The dam age of visual acuity was serious in most cases with eye injury complexty and man y complications. The management of operation in some cases was difficult. How to obtain the success of operation and visual progrnosis depends on factors such a s the damage degree of injured eyes, complications, the doctor's technique, surg ical instruments and the occasion of operation, etc.·
