振荡电位在糖尿病视网膜病变中的诊断价值

摘　　要：目的:用现代临床流行病学的方法评价视网膜电图的振荡电位(OPs)在糖尿病视网膜病变(DR)中的诊断价值。方法:选择89例(174眼)糖尿病患者进行散瞳眼底检查,按国内DR分期标准将患者分成眼底无改变组(组2),单纯型DR改变组(组3)和增生型DR改变组(组4)。选择34例(62眼)非糖尿病白内障术后正常眼作为对照组(组1)。对所有眼进行振荡电位的测定。结果:振荡电位的振幅总和(OPs)随着糖尿病视网膜病变的发展而下降。经t 检验,组3与组4,组1与组4和组2与组4的差异有统计意义(P <0.05)。振荡电位的信噪比(SNR)随着糖尿病视网膜病变的发展而下降。经t 检验,除组1与组2处于临界值外(P =0.076),各组间均值差异均有统计意义(P <0.05)。从ROC曲线上看,OPs曲线下面积为0.633,SNR曲线下面积为0.732,SNR的灵敏度、特异度都要高于OPs。结论:振荡电位的信噪比(SNR)比振幅总和(OPs)在糖尿病视网膜病变中的诊断价值更大。AIM: To assess the diagnostic value of the oscillatory potential of the elect roretinogram in diabetic retinopathy (DR) with modern clinical epidemiology.· METHODS: The summed amplitudes (OPs) and the signal-noise ratio (SNR) of the oscillatory potential of the electroretinogram were evaluated in a group of 89 d iabetic (174 eyes) and 34 control subjects (62 eyes) (Group 1). The diabetic pat ients were classified into three categories: no diabetic retinopathy (Group 2), nonproliferative diabetic retinopathy (Group 3) and proliferative diabetic retin opathy (Group 4), according to their retinopathy severity measured by fundus oph thalmoscope.· RESULTS: OPs reduced with the development of DR severity. The disparities bet ween Group 3 and Group 4, between Group 1 and Group 4 and between Group 2 and Gr oup 4 are statistically significant (P <0.05). SNR also reduced with the develop ment of DR severity. The disparities between each group were statistically signi ficant (P <0.05), except for the disparity between Group 1 and Group 2, which wa s on the marginal value (P =0.076). In the receiver operating characteristic cu rve (ROC curve), the area under the curve for the SNR test, which was 0.732, was greater than the area under the curve for the OPs test, which was 0.633.· CONCLUSION: The diagnostic value of the SNR of the oscillatory potentials in diabetic retinopathy is superior to that of the OPs.·
