急性运动性轴索神经病的病理与临床(附3例尸检报告)

摘　　要：本文报道了3例临床诊断为格林—巴利综合征(GBS)的尸检材料。一例为周围神经炎性节段性脱髓鞘等典型的GBS病理改变,另外两例以运动神经特别是前根轴索变性(瓦勒样变性)为主要改变的周围神经病,作者认为这是GBS的一种新亚型,称之为急性运动轴索神经病。结合临床及病理改变特点,对GBS、脊髓灰质炎及急性运动轴索神经病鉴别要点进行了讨论。This article reported autopsy data of 3 cases with Guillain — Barre syndrome (GBS) clinically defined. The pathological changes of one case were inflammatory segmental demyelination of peripheral nerves which was typical pathological changes in GBS. Other two cases were axonal degeneration (Wallerian — like degeneration) in motor nerves, particularly ventral roots. From the above findings,it is proposed that these cases belong to a new subgroup of GBS, termed acute motor axonal neuropathy (AMAN). The differential diagnosis for GBS, poliomyelitis and AMAN was briefly discussed.
