摘 要:目的探讨急性胰腺炎(acute pancreatitis,AP)中性粒细胞、炎性介质的动态变化以及在急性胰腺炎中的相互作用。方法SD大鼠分为对照组和AP组,每组18只,胰胆管逆行注射4%牛磺胆酸钠建立AP模型。按随机原则在制模后3,6,12 h分批处死,留取胰腺(体部)组织甲醛固定,HE染色光镜观察胰腺病理改变程度并进行病理学评分、免疫组织化学染色进行胰腺中性粒细胞特异性表面抗原Mac-1测定,心脏取血分离血清,供IL-1和TNF测定。结果随着制模后时间的延长胰腺病变程度加重,AP组在不同时间段胰腺中性粒细胞阳性细胞率、血清TNF及IL-1浓度均比对照组明显升高;随着时间的延长中性粒细胞阳性细胞率、血清TNF及IL-1浓度逐渐升高,3,6,12 h各时间段之间比较差异有显著性。在制模后3 h,Mac-1与TNF及IL-1为正相关(P<0.05),其余各指标之间无相关关系;在制模后6,12 h,各指标之间均为正相关关系。结论AP大鼠胰腺病变程度加重与中性粒细胞浸润加重、TNF及IL-1释放增加同步发生,并且呈密切正相关。中性粒细胞和炎性介质两者相互作用,互为因果共同加重急性胰腺炎病情发展。Objective: To investigate Dynamic change of PMN, inflammatory mediators and relationship between pancreas injury and them acute pancreatitis (AP). Methods: All rats were randomly divided into two groups, AP model was developed in adult rats by retrograde injection of 4% sodium taurocholate in the pancreatic duct. At the time of 3, 6, 12 h, get the pancreas for pathobiology and immunohistochemistry, get the serum for TNF and IL-1 by ELISA. Results: With the development of pancreatitis, PMN and TNF and IL-1 became more. Mac-1 had a positive relationship with inflammation mediators all the time (P <0.05), Mac-1 and inflammation mediators had positive relationship only at 6, 12 hours. Conclusion: Acute pancreatitis have intensely positive relationship with PMN and inflammatory mediators.
