胃癌术前小剂量化疗与细胞凋亡、bcl-2和p53蛋白表达及其关系探讨

摘　　要：研究胃癌术前小剂量化疗中细胞凋亡、bcl- 2和 p5 3蛋白的表达及其关系。 12 0例胃低分化腺癌患者 ,分为观察组 6 2例和对照组 5 8例 ,观察组术前静脉注射 5 -氟脲嘧啶 (5 -FU ) 5 0 0mg/天 ,共 3天。用TUNEL方法检测细胞凋亡 ,用免疫组化方法 (S -P法 )检测bcl- 2和 p5 3蛋白的表达。观察组于 2 4、4 8、72h的凋亡指数(AI)分别为 0 .11± 0 .0 2、0 .14± 0 .0 2和 0 .16± 0 .0 3,三者比较差异有显著性 (P <0 .0 1) ;对照组相应时间的AI则为 0 .0 8± 0 .0 1、0 .0 7± 0 .0 1和 0 .0 7± 0 .0 1,三者之间比较均无显著差异 (P >0 .0 5 )。观察组和对照组于 2 4、4 8和 72h ,bcl- 2蛋白阳性过度表达率分别为 5 0 .0 % /5 3.3%、2 3.3% /5 0 .0 %和 9.7% /4 8.3% ;而观察组和对照组于 2 4、4 8和 72h ,p5 3蛋白阳性过度表达率分别为 2 5 .0 % /2 1.4 %、33.3% /2 3.3%和 2 5 .8% /2 0 .7%。观察组和对照组相比bcl- 2的表达有显著性差异 (P <0 .0 5 ) ;而 p5 3表达则无显著差异 (P >0 .0 5 )。 5 -FU与胃癌细胞的凋亡有相关性 ,随治疗时间的延长能使bcl- 2蛋白表达逐步下调。p5 3蛋白高表达与胃癌的恶性程度和预后差有关。To investigate relationship of the apoptosis and expression of bcl-2 and p53 protein in gastric cancer with pre-operation chemotherapy, 120 patients were divided into two groups, 62 cases in treated group and 58 cases in control group.In treated group, patients were injected 5-FU into vein for 3 days. TUNEL method was used for gastric cell apoptosis and immunohistochemistry (S-P) was adopted to detect the expression of bcl-2 and p53 protein.Apoptosis index in 24, 48, 72 hour post-treatment was 0.11±0.02, 0.14±0.02 and 0.16±0.03 respectively in treated group, and 0.08±0.01, 0.07±0.01 and 0.07±0.01 respectively in control group.There was significant difference(P<0.01) in control group and no difference(P>05)in treated group.50.0%/53.3%,23.3%/50.0% and 9.7%/48.3% cases in two groups have the high expression of bcl-2 in 24,48,72 hour post-treatment, meanwhile 25.0%/21.4%, 33.3%/23.3% and 25.8%/20.7% cases in two groups have the high expression of p53 in 24, 48,72 hour post-treatment.Compared with control group, the expression of bcl-2 was significantly lower(P<0.05) in treated group, but there was no difference(P>05) in expression of p53 between two groups.The result revealed that there was a correlation between gastric cancer cell apoptosis and 5-FU.Meanwhile, there was correlation between 5-FU and bcl-2 protein expression.
