摘  要:目的 复制出产瘫在自然病程中较成年臂丛神经损伤更容易发生主动肌与拮抗肌同步收缩的动物模型。方法 取幼年和成年SD大鼠各12只,将右侧臂丛神经上干切断后,用硅胶管桥接使形成神经缺损3mm。1个月后显露并分别刺激C5、C6神经根做神经电图检查。结果 C5神经根刺激同时记录三角肌与大圆肌的诱发电位波幅峰值,在幼年两者的波幅比值为0.96±0.10,成年为4.54±2.70,两者差异有极显著性(秩和值T=202.0,P=0.0068)。C6神经根刺激同时记录肱二头肌与肱三头肌的诱发电位波幅峰值,幼年比值为0.99±0.11,成年为3.66±2.19,两者差异有极显著性(秩和值T=222.0,P=0.0004)。结论 臂丛神经上干损伤后在自然恢复过程中,幼年大鼠较成年容易发生主动肌与拮抗肌的同步收缩,因此大鼠是研究产瘫后同步收缩发生机制的适宜动物。Objective To establish the animal model of the more frequent occurrence of co-contraction after obstetric brachial plexus injuries than in the adult with brachial plexus lesions during the natural healing process. Methods Twelve specific pathogen free (SPF) 18-day-old Sprague-Dawley rats (called as young group as followed) each weighing approximately 38-42 g were chosen to correspond as nearly as possible to humans of about 1 year of age. Twelve adult rats of 90-100 days weighing 200-250 g (called as adult group as followed) were chosen to correspond as adult humans. The nerve defect (3 mm in length) was bridged with a silicone tube after severance of upper trunk of brachial plexus at the right side. One month later, the rats were all re-anesthetized for EMG examination. The C5 nerve root was mobilized and stimulated by the sterilized mono-thread electrode. Two concentric needle electrodes were inserted into the deltoids and teres major muscles simultaneously to record the firing of motor units and amplitude of compound muscle action potential (CMAP). Similarly, the C6 nerve root was mobilized and stimulated, and the firing of motor units and amplitude of CMAP at the biceps and triceps was recorded simultaneously. The amplitude ratio of deltoids to teres major, and of biceps to triceps was calculated both in the young and adult rats, respectively. Significant difference for the data analyses was evaluated by Wilcoxon test. The criterion for statistical significance was considered to be P<0.05. Results When C5 nerve root was stimulated, the CMAP amplitude of the evoked potentials in the deltoids and teres major was recorded, The ratio of amplitude between deltoids and teres major was 0.96±0.10 in the young rats and 4.54±2.70 in the adult rats respectively. There was statistically significant difference (Sum of score T=202.0, P<0.01). Similarly, when C6 was stimulated, the CMAP amplitude of the evoked potentials in the biceps and triceps was recorded. The ratio of amplitude between biceps and triceps was 0.99±0.
