扫描式视网膜厚度分析仪(RTA)在青光眼早期诊断中的价值

摘　　要：目的　评估以色列TALIA -RTA激光眼底扫描仪、眼电生理仪以及全自动静态视野计在原发性青光眼早期诊断中的意义。方法　选择早期原发性青光眼患者 10 6例 190只眼和正常对照组 49人 98只眼 ,分别用RTA激光眼底扫描仪、眼电生理仪以及全自动静态视野计进行黄斑部平均视网膜厚度、视盘参数、PERG、PVEP和中央视野视网膜光敏感度的测定 ,比较这些方法的敏感性和特异性。结果　 1 两组人群在视盘面积、视杯容积、平均视杯深度方面无显著差异 (P >0 0 5 ) ,而在其余视盘参数如 :视杯面积、视杯 /视盘面积比、盘沿面积等的比较中均有显著性差异 (P <0 0 1)。 2 两组人群在视盘区、视盘周围区、黄斑区、视盘黄斑束的平均视网膜厚度值均有显著性差异 (P <0 0 1)。 3 诊断指数 :PERG >RTA >PVEP >视网膜光敏感度。结论　PERG、RTA的敏感性和特异性高于其它方法 ;联合多种检查方法对原发性青光眼的早期诊断具有较大的意义。Objective To evaluate the clinical values of ISRAEL TAIIA Retina Thickness Analysis(RTA),pattern electroretinagraphy(PERG),pattern reversal visual evoked potential(P-VEP)and the visual field(VF)on the early diagnosis of primary glaucoma(PG).Methods 106 patients(190 eyes)with early PG and 98 normal eyes were examined by RTA,PERG,PVEP and VF.The susceptibility and peculiarity of four methods were statistically measured and inspected.Results There were no singnificant differences between two groups in Disk Area,Cup volume and Mean Cup Depth(P>0.05).The differences between two groups in other indexes of optic disk,mean retina nerve fibers layer thickness in optic disk area,disk round area,posterior-pole area and papilla macular never fiber bundle were significant(P<0.01).The order of clinical value for the early diagnosis of PG in four methods was PERG>RAT>PVEP>VF.Conclusions PERG,RTA are more sensitive in the early diagnosis of PG.It is valuable in combination of many tests for the early diagnosis of PG.
