老年人消化溃疡出血的临床特点及内镜注射治疗

摘　　要：目的　探讨老年人消化性溃疡出血的临床特点及内镜注射治疗的效果。方法　采用分组对照的方法对两组临床和内镜的特点以及治疗结果进行分析。结果　内镜注射止血成功 78例 ,2例内镜止血失败后成功的进行了外科手术。老年组患者在出血量、2 4h内输血次数、慢性病史及病变部位与对照组比较有显著性差异。结论　急诊内镜检查是明确消化性溃疡出血病因的首选方法 ,采用镜下止血安全、有效、经济。Objective To investigate the clinical characteristics of upper GI bleeding from the peptic ulcer and endoscopic injection therapy in aged men.Methods Patients were divided into elderly group and comparison group.Data were analyzed with respect to clinical and endoscpic characteristics as well as outcome.Results Endoscopic injection therapy was successful in 78 patients(97.5%).Two patients had successful surgery after endoscopic therapy failure. The elderly men had severe acute upper gastrointestinal bleeding.There was significant difference between the 2 groups in bleeding volume (P<0.01),location of lesion (P<0.01),number of therapeutic endoscopic sessions (P<0.01),chronicity of diseases (P<0.01) and number of blood transfused(P<0.01). There were no deaths due to bleeding and no endoscopic complications.Conclusions Emergency endoscopic dignosis is possible in all patients with acute upper gastrointestinal bleeding.The aged males are more frequently with severe acute upper bleed gstrintestinal bleeding.Endoscopic injection therapy in treating upper gastrointestinal bleeding is safe,successful,economical,effective,and should be recommended.
