Array多焦点人工晶状体的临床应用

摘　　要：目的　评价白内障患者植入多焦点人工晶状体后的视功能以及并发症。方法　将 10 1例 (13 7眼 )行白内障超声乳化术并植入不同类型人工晶状体的患者随机分为二组 ,Ⅰ组为多焦点人工晶状体植入组 (简称多焦组 ) :5 0例 (71眼 ) ,植入ArraySA -4 0N多焦点人工晶状体 ,平均随访时间 17 68± 4 42月 ;Ⅱ组为单焦点人工晶状体植入组 (简称单焦组 ) :5 1例 (66眼 ) ,植入SI -4 0N单焦点人工晶状体 ,平均随访时间 2 3 0 4± 4 70月 ,观察术后裸眼及最佳矫正远、近视力以及并发症。结果　两组患者术后裸眼远视力 ,最佳矫正远视力和最佳矫正近视力均无显著性差异 (P >0 0 5 )。Ⅰ组术后裸眼近视力和最佳视远度数矫正下的近视力明显优于Ⅱ组 (P <0 0 0 1) ,术后裸眼近视力≥ 0 5者在Ⅰ、Ⅱ组中分别为 85 9%和 16 7% ;最佳视远度数矫正下的近视力≥ 0 5者在Ⅰ、Ⅱ组中分别为 90 1%和 18 2 %。两组均无严重的术后并发症。结论　Array多焦点人工晶状体可为患者提供良好的远距及近距视力 ,并在很大程度上摆脱对眼镜的依赖性 。Objective To evaluate the visual functiona and complications after phacoemulsification combined with implantation of a multifocal intraocular lens(MIOL) in patients with senior cataract.Methods 101 patients(137 eyes)with senior cataract were divided into two groups,Group Ⅰ:50 cases(71 eyes),Array SA-40N MIOL was implanted after phacoemulsification,the average time of follow-up was 17 68±4 42 months;Group Ⅱ:51 cases(66 eyes),SI-40N monofocal intraocular lens(SIOL)was implanted after phacoemulsification,the average time of follow-up was 23 04±4 70 months.The uncorrected and best-corrected distance and near visual acuities and complications after operation were observed.Results Group Ⅰ had better uncorrected near vision and best distance-corrected near visual acuity than Group Ⅱ( P <0 001),uncorrected distance visual acuity,best-corrected distance visual acuity and best-corrected near visual acuity in Group Ⅰ was similar to Group Ⅱ( P >0 05).A higher proportion in the multifocal group achieved 0 5 or better uncorrected near visual acuity and 0.5 or better best distance-corrected near visual acuity than in the monofocal group(85.9% versus 16.7% and 90.1% versus 18.2%).There have been no serious postoperative complications in either group.Conclusions The results of this study indicate that Array multifocal IOL implantation may provide a better way to help patients with cataract to obtain satisfying distance visual acuity,better uncorrected near visual,less spectacle dependency and high quality of life.
