超声引导下麦默通负压抽吸活检仪在切除乳腺良性肿块中的应用

摘　　要：目的　探讨超声引导下麦默通负压抽吸活检仪在切除乳腺良性肿块中的应用价值。方法　 32例经临床与超声诊断为乳腺良性肿块的女性患者 ,在超声引导下行乳腺肿块切除术 ,肿块径线≥ 1.5 cm的患者 ,采用 8G取样器进行切除 ,肿块径线 <1.5 cm的患者 ,采用 11G取样器进行切除。术后 1个月进行随访。结果　超声清晰地显示了取样全过程 ,显示率 10 0 %。 32例患者中 ,肿块径线≥ 1.5 cm的患者 19例 ,其中肿块径线 1.5～ 3.0cm的患者 16例 ,肿块完全切除者 14例 (87% ) ;肿块径线 3.0～ 3.5 cm的患者 3例 ,肿块完全切除者 0例。肿块径线 <1.5 cm的患者 13例 ,肿块完全切除者 12例 (92 % )。 32例患者无 1例有切口疤痕。术后仅 1例有较严重地出血。结论　超声引导下麦默通负压抽吸活检仪切除乳腺良性肿块具有穿刺准确、创伤小、操作简单、安全、疗效好等优点 ,尤其适合于 3.0 cm以下的乳腺小肿块 。Objective To explore the clinical value of minimally invasive complete excision of benign breast lesions using ultrasound-guided mammotome vacuum device.Methods Thirty-two women with benign breast lesions were studied with ultrasound-guided mammotome vacuum deviceLesions ≥15 cm in size were removed using an 8-gauge probeThose lesions of 15 to 30 cm in size were removed using an 11-gauge probeFollow-up evaluation was performed in 1 month after biopsy.Results The needle position in thirty-two patients was all real time visualized (100%)Lesions ≥15 cm in size were found in 19 patients;lesions of 15 to 30 cm in 16 patients;lesions of 30-35 cm in 3 patients.Lesions were completely removed in 14 patients with sizes 15 to 30 cm(87%)Lesions with 30-35 cm were not completely removedLesions<15 cm in size were found in 13 patients and were completely removed in 12 patients(92%).Postoperative cutaneous scar were not found.One patient had postoperative bleeding required a second operation to ligate the vessel.Conclusions Excision of benign breast lesions using ultrasound-guided mammotome vacuum device is minimally invasive,safe,and feasible and especialy suitable for benign breast lesions ≤30cm in sizeIt is a good method for cosmetology.
