摘  要:目的:观察功能性电刺激合并早期康复对脑卒中偏瘫患者下肢运动功能的改善作用及其安全性,并探讨其可能机制。方法:选择112例吉林大学第二临床医学院神经内科确诊后转入康复科的急性脑卒中患者,随机分为治疗组56例和对照组56例。治疗组采用Bobath和运动再学习法、作业疗法、功能性电刺激相结合的方法进行训练,对照组只施行单纯的早期康复治疗。结果:治疗组治疗后Fugl-Meyer的下肢运动功能积分为22.00±3.37,与治疗前5.82±2.33比较有显著提高(P<0.05),对照组治疗后Fugl-Meyer的下肢运动功能积分为14.43±3.21,与治疗前6.09±2.13比较也有一定的提高,但不如治疗组,治疗后两组间差异有显著性意义(P<0.05)。结论:功能性电刺激合并早期的康复治疗,对促进偏瘫患者下肢运动功能积分的恢复,提高患者的步行和生活自理能力,提高生活质量,预防继发损害有重要意义。AIM:To observe the effects and safety of functional electrical stimulation(FES) combined with early rehabilitation on the integral of motor function of lower limbs in patients with stroke hemiplegia,and investigate the possible mechanism. METHODS:A total of 112 patients with acute stroke were selected from the Department of Rehabilitation,Second Clinical Medical College of Jilin University after diagnosed as having stroke in the Department of Neurology.All patients were randomly divided into two groups,including FES combined with early rehabilitation group(n=56) and simple early rehabilitation group(n=56).Patients in treatment group were treated with the combination of Bobath's and motor relearning program(MRP),occupational therapy and FES.Patients in control group were treated with the simple early rehabilitation. RESULTS:The integral of Fugl-Meyer on the motor function of lower limbs was 22.00±3.37 after treatment in treatment group,which was significantly higher than that before treatment(5.82±2.33)(P< 0.05).The integral of Fugl-Meyer was 14.43±3.21 after treatment in control group,which was improved compared with that before treatment(6.09±2.13),but lower than that in treatment group.It was significantly different between the two groups after treatment(P< 0.05). CONCLUSION:FES combined with early rehabilitation is very important in accelerating the recovery of motor function of lower limbs,improving the ability of walking and activities of daily living,ameliorating the quality of life,and preventing secondary disability in patients with hemiplegia.
