Herbst矫治器与头帽Herbst矫治器治疗AngleⅡ类错的临床比较

摘　　要：目的 :观察Herbst矫治器与头帽Herbst矫治器对生长期AngleⅡ类错牙合患者牙牙合、颅面的影响。方法 :选取青春生长期的安氏Ⅱ类错牙合患者 2 1例 ,其中 13例采用Herbst矫治器 ,8例采用联合头帽的Herbst矫治器治疗。通过治疗前后头影测量分析 ,对所产生的颌骨、牙牙合、软组织侧貌改变进行比较。结果 :所有患者侧貌均改善 ,磨牙关系达Ⅰ类 ,覆盖和覆牙合明显减小。头帽Herbst组SNA、ANB减小较显著 ,而Herbst组下切牙 -下颌平面夹角增大明显。头帽Herbst组牙合平面表现更显著的顺时针旋转 ,而前面高、下面高增加在Herbst组更显著。Herbst组下颌骨长度变化更显著 ,而软组织变化两组无明显差异。结论 :两种矫治器均明显改善患者颜面侧貌、磨牙关系、覆盖和覆牙合 ;头帽Herbst能更有效地在矢状和垂直方向上抑制上颌骨的生长 ,而单独使用Herbst矫治器更多地促进下颌的生长。Objective: To investigate the skeletal and dental effects contributing to the corrections of Class Ⅱmalocclusions in growing patients treated with Herbst appliances and headgear-Herbst appliances. Methods: The investigation involved 13 patients treated with Herbst appliances and 8 patients treated with headgear-Herbst appliances. The lateral cephalograms were analyzed before the treatment and immediately after the treatment. Results: All subjects achieved to ClassⅠ molar relationships. Patients in headgear-Herbst group showed more decrease in angle SNA, angle ANB, and more clockwise rotation in occlusal plane. The increase in anterior facial height, low facial height, and the mandibular length were larger in Herbst group. The effects on the lateral profiles were similar in both groups. Conclusion: Both kinds of appliances may improve the profiles, correct the molar relationships and reduce the overbite and overjet in patients with Class Ⅱ malocclusions. The headgear-Herbst appliance improved jaw base relationship more by the restraint of forward and downward growth of the maxilla, while Herbst appliance alone more by improvement of the growth of the madibule.
