OUR旋转式伽玛刀治疗听神经瘤23例临床分析

摘　　要：背景与目的:听神经瘤是桥小脑角(C-P 角)区常见的良性肿瘤,以往治疗多以手术切除为主。近年来,听神经瘤的伽玛刀治疗逐年增多。本文主要探讨 OUR 旋转式伽玛刀治疗听神经瘤临床疗效。方法:采用 OUR旋转式伽玛刀治疗23例听神经瘤患者,观察术后病情变化。结果:本组伽玛刀治疗病例随访6个月～36个月,其中肿瘤缩小12例(52.17%),肿瘤体积不变9例(39.13%),肿瘤增大2例(8.69%),肿瘤控制率91.3%,面神经保留率95.65%,听力保留率78.3%。结论:伽玛刀治疗听神经瘤的疗效肯定,损伤小,并发症少,能有效提高患者生存质量。BACKGROUND & OBJECTIVE:Acoustic neuromas are the most common benign tumors in cerebello-pontine angle.Microsurgical excision is an established treatment for acoustic neuromas.Treatments of acoustic neuroma by Gamma Knife become popular nowadays.In this paper,the therapeutic effect of acoustic neuro- ma by OUR rotated Gamma Knife was summarized.METHODS:Twenty-three patients with acoustic neuromas were treated by Gamma knife from 1999 to 2003.All patients were followed up for 6-36months.RESULTS:Neu- roimaging studies showed that tumor size decreased in 12 cases(52.17%),unchanged in 9 cases(39.13%), and increased in 2 cases(8.69% ).The tumor growth control rates were 91.3%.Facial nerve was preserved in 22 patients(95.65%).Functional heating was preserved in 18 patients(78.3%)who had useful hearing prior to Gamma knife treatment.Serious complications were observed in one patient,but no patient died.CONCLU- SIONS:These findings indicate that Gamma Knife is an effective for acoustic neuroma with relatively little injury and complication.
