佳代胞对肺癌术后免疫功能调节的临床观察

摘　　要：30例肺癌术后胸腔内注射假单胞菌 (pseudomonasjinanensisvaccine ,PJV) ,对照组 2 9例 ,分别于手术前、后测定T淋巴细胞亚群 (CD3 + 、CD4+ 、CD8+ 、CD19+ )、自然杀伤细胞 (NK)、血清免疫球蛋白 (IgG、IgA、IgM )含量 ,对比观察变化及与临床病理因素的关系。实验组和对照组术前与术后血清免疫球蛋白IgG、IgA和IgM无显著变化 ;实验组术后的CD3 + 、CD4+ 、CD19+ 分别为 68 93 %、3 9 0 7%、12 5 4% ,高于对照组 66 86%、3 4 68%、10 0 9% ;CD8+ 为 3 0 92 % ,低于对照组 3 7 99% ,差异有统计学意义 ,P <0 0 5。术前ⅢA期与Ⅰ、Ⅱ期比较 ,CD4+ 、CD19+ 、NK分别为 3 3 2 4%、8 2 5 %、12 2 9% ,有下降趋势 ,差异有统计学意义 ,P <0 0 5。淋巴结转移N1组与N0 组比较 ,CD4+ 、CD8和CD19+ 差异有统计学意义 ,P <0 0 5。不同病理类型组的T细胞亚群、NK细胞无论在术前还是术后相比较差异均无统计学意义。The subsets (CD 3+,CD 4+,CD 8+ and CD 19 +) of T lymphocyte, NK cell and the serum level of immunoglobulins from peripheral blood were detected during the perioperative period in 30 postoperative patients with lung cancer receiving intrapleural PJV and no administration in 29 cases of the control, and the relationship between the serum level and clinical materials was evaluated. As compared with those before operation, the serum level of IgG, IgA and IgM had no obvious changes at the postoperative days; CD 3+, CD 4+ and CD 19 + of the postoperative PVI group were 68.93%, 39.07% and 12.54% respectively and was significantly higher than those in the control group,but CD 8+ was 30.92% and lower than that of the control (37.99%),P<0.05. There were significant differences among preoperative stage ⅢA, Ⅰ and Ⅱ, and CD 4+, CD 19 + and NK were 33.24%, 8.25% and 12.29% respectively, and there was a tendency of decrease in the subsets and NK cells,P<0.05. There were significant differences in the changes of CD 4+,CD 8 and CD 19 + between preoperative N 1 diseases and N 0 group,P<0.05. There was no significant correlation between the perioperative changes of the subsets and NK cells and pathological type.
