摘 要:目的 研究细胞周期抑制蛋白p14 ARF在非小细胞肺癌 (NSCLC)中的表达及其临床意义。方法 应用免疫组织化学S P方法检测 10 3例NSCLC癌组织标本中p14 ARF蛋白表达 ,同时以 2 0例肺部炎症病变组织作对照。结果 10 3例NSCLC标本中 ,p14 ARF阳性表达 3 0例 ,阴性表达 73例 ,表达阴性率为 70 .87%。不同类型NSCLC中p14 ARF表达不同 :鳞癌中p14 ARF表达阴性率较高 (88.2 4% ) ,肺泡癌中较低 (5 2 .78% ) ,腺癌介于两者之间 (72 .73 % ) ,各组织类型之间的差异有非常显著的意义 (P <0 .0 1)。临床Ⅲ +Ⅳ期病例p14 ARF蛋白表达阴性率为 78.5 7% ,明显高于临床Ⅰ +Ⅱ期 (5 4.0 5 % ) ,差异有显著性意义 (P <0 .0 5 ) ;男性患者癌组织中p14 ARF阴性率 81.94% (5 9/72 )明显高于女性 45 .16% (14 /3 1) ,差异有显著性意义 (P <0 .0 1)。结论 NSCLC组织p14 ARF蛋白表达异常具有明显的组织学类型特异性 ;p14 ARF蛋白表达异常在NSCLC发生、发展中起重要作用 ,有可能同时成为预测病程进展的指标之一。Objective To study the expressions and clinical significances of cell cycle control protein p14 ARF. in non-small cell lung cancer (NSCLC).Methods The expression of p14 ARF. protein was detected in 103 NSCLC samples by immunohistochemistry (S-P method),and 20 benign masses of lung as control.Results The negative expression of p14 ARF. was detected in 73(70.87%)of 103 NSCLC cases.30 cases showed positive immunohistochemical result.The negative rates of p14 ARF. expression in squamous cell carcinoma,adenocaricinoma and bronchioloalveolar carcinoma were 88.24%(30/34),72.73%(24/33) and 52.78%(19/36) respectively,and the significant difference was observed between the three groups (P<0.01).Of the 65 samples analyzed from NSCLC patients,the negative rates of p14 ARF. expressions were 54.05%(20/37) and 78.57%(22/28) in the groups of clinical stage Ⅰ+Ⅱ and Ⅲ+Ⅳ respectively.p14 ARF. negative expression was closely related to clinical stage(P<0.05);The negative rates of p14 ARF. expressions were 81.94%(59/72) and 45.16%(14/31) in the groups of male and female respectively.p14 ARF. negative expression was closely related to sex(P<0.01).Conclusion The negative rates p14 ARF. expressions was very high,which points to p14 ARF.,might play an important role in the onset and development of NSCLC.Moreover,the negative rates of p14 ARF. expressions in squamous cell carcinoma was significantly higher than that in other pathological types,it indicated that expression alteration of ARF gene had the tendency of histopathological specificity.Patients with higher clinical stages tended to show significantly higher negative rates of p14 ARF. expression,it was suggested that evaluation of p14 ARF. expression in NSCLC might be useful as a prognostic factor.
