摘　　要：目的:探讨认知心理和认知行为干预对心脏瓣膜置换术患者围手术期负性情绪的影响。方法:将泸州医学院附属医院胸心外科择期行心脏瓣膜置换术患者80例,随机分为治疗组和对照组,每组40例。治疗组给予综合性心理行为干预,对照组只作一般性解释。分别于干预前、干预后2d、1周、2周、3周采用焦虑自评量表(SAS)和抑郁自评量表(SDS)进行测评。结果:入院时80例患者均存在焦虑或抑郁等负性情绪;治疗后治疗组发生焦虑、抑郁人数明显少于对照组,差异有显著性意义(χ2=4.025～8.350,P=0.003～0.044);SAS,SDS评分显著低于对照组,差异均有显著性意义(t=3.460～6.219,P均<0.001)。结论:心理行为干预能较好的改善心脏瓣膜置换术患者围手术期负性情绪,有利于围手术期患者的康复。AIM:To investigate the influence on the negative emotion by cognitive psychological and behavioral interventions in patients with heart valve replacements during the perioperative period. METHODS:Eighty patients, who received heart valve replacements in the Department of Cardiothoracic Surgery,Affiliated Hospital of Luzhou Medical College,were randomized into treatment group(n=40) and control group (n=40).They were randomly divided into two groups:trial group and control group.Patients in the treatment group were given comprehensive psychological and behavioral interventions,while those in the control group received only general explanation.Self-rating anxiety scale(SAS) and self-rating depression scale(SDS) were used to evaluate the patients in both groups before,and 2 days,1,2 and 3 weeks after intervention respectively. RESULTS:All the patients had emotional disorders of anxiety and depression when they were hospitalized.The number of patients with anxiety and depression after treatment was significantly less in the treatment group than in the control group(χ2=4.025 to 8.350,P=0.003 to 0.044).The scores of SAS and SDS in the treatment group were remarkably lower than those in the control group(t=3.460 to 6.219, P all < 0.001). CONCLUSION:Comprehensive psychological and behavioral interventions can better improve the negative emotion in patients with heart valve replacements during perioperative period,and it is good for their rehabilitation.
